WHILE HIS 777 END DUMP TRUCK WAS BEING LOADED, EE REACHED BEHIND THE SEAT FOR PAPER TOWELS AND STUCK HIS FINGER IN THE HEATER VENT. HE WAS TAKEN TO THE LOCAL E.R. WHERE HE RECEIVED 5 STITCHES AND WAS GIVEN A PRESCRIPTION DORVOCET AND RELEASED TO WORK AT RESTRICTED DUTY.
AT 0530 LOADER #I-62 CAUGHT ON FIRE. 5 HANDHELD FIRE EXT. WERE USED BUT WERE UNSUCCESSFUL. THE LOCAL FIRE DEPT RESPONDED & THE FIRE WAS EDXTINGUISHED AT APPROX 0615. THIS WAS REPORTED TO MSHA AT 0630 AND THE SCENE PRESERVED. CAUSE OF FIRE UNKNOWN AT THIS TIME. DAMAGES: TOTAL LOSS.

EE WAS LOOSENING BOLTS ON A PUMP LOCATED AT THE PREPARATION PLANT USING ON IMPACT WRENCH. THE AIR CREATED BY THE WRENCH CAUSED ON UNKNOWN PARTICLE TO COME IN CONTACT WITH EE'S LEFT EYE. EE'S INJURY WAS EXAMINED ADN DIAGNOSED AS A CORNEAL ABRASION & GIVEN A PRESCRIPTION GENTAMICIN SULFATE OPTHAMALIC SOLUTION AT THE LOCAL HOSPITAL ER.
WHILE WORKING IN PREP PLANT, EE WAS WALKING AROUND CLEAN COAL BELT, HE CAUGHT HIS FOOT ON A METAL ROD & FELL TO THE FLOR, TWISTING HIS RIGHT KNEE. AT THAT TIME, NO LOST TIME. ON 1/10/99 EE HAD DR APPT & SURGERY WAS SCHEDULED TO BE PERFORMED 1/25/99. AS OF THIS DATE, HE HAS NOT RETURNED TO WORK.
