Employee was cutting an old section of conveyor belt. EE cut two fingers with a utility knife. Employee received stitches to ring finger. EE returned to work with restrictions. EE is scheduled for a follow up on Monday 7/22/24.

Haul truck hit a bump and the right glass window broke. Employee grabbed right armrest leaned to the left to avoid the glass, hand slipped off causing a small laceration to EE's right arm. Employee was taken to the hospital for evaluation. The wounds were cleaned & sutured. The employee was released from ER with No Work Restrictions.

Went to get into the Haul truck, stepped up on the foot board reached for the hand bar, hand bar had frost on it. EE's hand slipped causing EE to fall to the ground.

An aggregate laborer was attempting to loosen a cap on a fuel tank with a pipe wrench when felt pain in back. The laborer finished out the day doing normal duties and returned to work the next day and went about normal duties. At the end of the second day employee decided to seek medical treatment and was put on restricted duties for lower back pain.

While installing a pipe section on the plant slurry pump, EE attempted to lift into place a section of pipe. He slipped in the mud & caught his fingers on his left hand, between 2 pieces of pump, resulting in 2 broken fingers on left hand.

EE WAS DRIVING THE SKID STEER ACROSS THE YARD AND HIT A POTHOLE. IT THREW HIM AROUND IN THE CAB AND STRAINED HIS NECK SO WE SENT HIM TO HAVE IT CHECKED OUT JUST TO BE ON THE SAFE SIDE.
EE WAS LOADING SCRAP STEEL ON THE BACK OF THE TRUCK. HE THREW ONE PIECE OF METAL ON THE TRUCK AND SMASHED HIS RING FINGER ON HIS LEFT HAND BETWEEN TWO PIECES OF STEEL.
Employee was opening a bucket of crater compound grease. The screwdriver he was using slipped out of the notch on the bucket lid and he hit his finger on the bucket ear. He cut his middle finger on his right hand.

EMPLOYEE SAID HE WAS PUSHING MUD OFF THE CONVEYOR BELT WITH A RAKE WHEN HE HURT HIS BACK.
HE IS UNSURE OF EXACTLY WHEN THE ACCIDENT HAPPENED. HE WAS HELPING REPAIR THE PLANT BY REPLACING A 10 HP MOTOR AND HELPING UNPLUG THE FLUME UNE.
EE IS NOT SURE IF HE HURT HIS BACK WHILE DRIVING IN THE END DUMP OR IF HE HURT IT WHILEHE WAS TWISTING OUT THE WINDOW TO UNLOAD THE PLANT BINS.
AFTER STEAM CLEANING UNIT 104 EE PULL THE TRUCK BACK INTO THE SHOP FOR REPAIR. EE STEPPED OUT OF THE TRUCK ONTO THE STEP ON THE FLIER TANK AND SUPPED. FALLING ONTO HIS BACK.
INJURED CLAIMS THAT HE SLIPPED ON THE STEP OF HIS TRUCK AS HE WAS CLIMBING BACK IN. HE SAID HE HURT HIS RIGHT SHOULDER.
EE IS NOT SURE WHEN THE INJURY HAPPENED, BUT CLAIMS IT MUST HAVE HAPPENED WHEN HE WAS DIGGINGIN THE BANK DOWN IN THE PIT ON SAT.MAY 16 1998. THIS INCIDENT WAS REPORTED TO ME AS A WORK RELAT ED INJURY UNTIL 1PM ON 5/21/98

AFTER HIS BOTTOM DUMP TRAILER WAS LOADED BY THE FRONT END LOADER HE PULLED UP APPROX 100 YDS, GOT OUT OF TH E TRUCK & DROPPED TO HIS KNEES. AFTER A FEW MINUTES HE GOT UP & WALKED ANOTHER 50' & DROPPED. THE LOADER OPERATOR RADIOED THE OFFICE TO CALL 911
WHILE HELPING TO TURN BLOW BARS IN THE K-0 HAZEMAG CRUSHER & AFTER SUCCESSFULLY TURNING 2 OF THE4 BARS EE SAID HE HEARD A POP IN HIS BACK & FELTPAIN IN HIS BACK WHILE HELPING TO LIFT THE 3RD B AR INTO PLACE.
