Employee was cleaning catwalk and turned sending intense pain to EE's foot/ankle area making it hard to move and walk. Employee has had foot discomfort on left foot for a couple weeks and never told anyone. Employee was seen in ER, was given an injection for pain, and taken out of work for 1 day. EE was diagnosed with tendonitis.

While driving loader, operator ran over a big stone that caused operator to be jolted inside the cab. While steadying self, operator placed left hand on the frame of the cab (where door hinges are). The door swung closed and pinned operator's thumb. Thumb was fractured, received 2 stitches, and given prescription antibiotics.

Injured worker was team lifting a 60lb sheave when the sheave slipped out of coworker's hands and dropped on IW's right hand. When the sheave was dropped it pinched IW's right hand between the sheave and some steel guarding in the area that resulted in IW receiving 4 stitches to the right ring finger. Injured worker was placed on light duty for 2 weeks.

Unplanned fire on the property was observed at 8:30pm. Fire dpt was dispatched at 8:30pm. Site personnel confirmed dry tower area/belting was on fire at 9:15pm. Fire was extinguished by local fire dpt around 9:30pm. Hot work activities that occurred on the dry tower area around 7:30pm are believed to be a contributing factor. Last employee left the site around 7:53pm.

Injured worker was bent over walking under a belt, stepped on uneven ground, rolled ankle, and almost fell. While trying to catch self from falling the worker strained lower back. The worker did not actually fall. The worker notified supervisor 2 days later, went to the ER where worker was given a prescription for a muscle relaxer and taken out of work for 5 days.
