Employee descended steps to primary hopper and after turning around lost footing and hyperextended left leg.

While throwing a rock into the jaw to help get a big rock out of the crusher employee's hand was smashed between the rock and the I beam in the front of the opening. This caused the skin to open on left thumb.

While placing a screen panel on the catwalk another screen scraped EE's right forearm causing lacerations, requiring 19 stitches.

Employee was cutting cardboard with a straightedge and a razor knife. Knife slipped over straightedge and cut thumb of left hand.

He parked loader and was trying to put window up from platform and he stepped back his right foot got stuck in the top step and he fell to ground. Right ankle fractured in 2 places and crushed heel.

2 workers were welding, cutting and grinding on the screen deck to the wash tower and at lunch one worker's eyes started watering and burning.

Employee was removing bolts from the large cover for the blend chute, as he was taking the last bolt out it came down and hit his left thumb and cut it. He received 6 stitches.

EE was trying to put safety pin in slot of rotor when it loosened up enough for the rotor to slide and pinch his finger between the rotor and the wall of the crusher.

WHEN DOING MAINTENANCE ON PLANT, EMPLOYEE WAS CHANGING A SCREEN & IT SLIPPED FROM HIS GRASP & SLID DOWN HIS ARM & PUT A GASH IN HIS RIGHT ARM.

EMPLOYEE STATES THAT WHILE SHOVELING DUST FROM THE CENTER OF THE CATWALK HE OVER-EXTENDED HIS REACH AND PULLED A MUSCLE IN THE CENTER OF HIS BACK.HE WAS NOT LIFTING ANY HEAVY MATERIAL OR PARTS .

EMPLOYEE STATES HE WAS WELDING ON HAMMERS AT WELDING TABLE & HE TURNED AROUND & WHEN HE T URNED HE BUMPED THE TABLE CAUSING HAMMER TO FALL

EMPLOYEE WAS ATTEMPTING TO PULL OUT FLOP DOORS ON CONVEYOR BELT WHEN HE FELT A TWINGE IN HIS BACK

EMPLOYEE WAS GETTING SAMPLES AT VAIR VILEE AND TURNED AROUND AND HIT A BEAM CAUSING A GASH OVER RIGHT EYE JUST ABOVE BROW.

EMPLOYEE WAS PICKING UP A PIECE OF PLASTIC PIPE & PULLED MUSCLES IN HIS BACK. WAS NOT AWARE OF LOST TIME UNTIL 7-11-89, INCIDENT WAS REPORTED AS FIRST AID ONLY.

EE WAS INJURED 4HEN A 50 TON JACK WAS SUPPORTING A 35 TON EUCLID HAUL UNIT FAILED, CAUSING A CROWBAR THAT THE EE HAD BEEN WORKING WITH TO GRAZE HIM ON THE LEFT SIDE OF HIS SCALP LACERATING T HE SKIN. 3

EMPLOYEE WAS DISMOUNTING FROM A FRONT END LOADER WHEN HE SLIPPED COMING DOWN THE LADDER AND FELL TO THE GROUND, LANDING ON HIS BACK.

EMPL LIFTING STEEL PLATE AND FELT SOME STRAIN.AN INJUINAL HERNIA RESULTED.EMPL DIDNT MENTION INJURY TO FOREMAN UNTIL 1 WK AFTER INCIDENT TOOK PLACE.NEXT REPORT FROM EMPL WAS DAY AFTER HE VISIT ED DR.5-7-84.CONTINUED TO WORK UNTIL 5-15-84.
