Struck against a moving object

Employee was hauing shot rock to crusher from pit. Employee went to wipe sweat off his glasses and ran off the road. Truck went up berm and turned over on passenger side.

Over-exertion in lifting objects

Employee was helping mechanic move some 10"" x 10"" x 4"" wood blocks that was blocking tires while he was using to chock wheels. He said that he felt something pull but did not think it was anything until next day. Sent him to the Dr. They said pulled muscle. Then on 5/16/07, he went back and they found a hernia.

Contact with heat

Working in heat all day while driving home started feeling sick. Once home he decided to go to the hospital that evening around 11pm. At the time they said he had heat exhaustion and should be off work the next day.

Fall from machine

EE stood up to get another employee attention and forgot the door was open and fell. When EE fell she hit her side on the floor which is what broke her ribs.

Employee was replacing flashing when electric drill slipped and drill bit contacted employee's finger.

Employee was lifting a wood block when he pulled a muscle. The result of this was improper lifting techniques.
