Employee was helping mechanic move some 10" x 10" x 4" wood blocks that was blocking tires while he was using to chock wheels. He said that he felt something pull but did not think it was anything until next day. Sent him to the Dr. They said pulled muscle. Then on 5/16/07, he went back and they found a hernia.

Working in heat all day while driving home started feeling sick. Once home he decided to go to the hospital that evening around 11pm. At the time they said he had heat exhaustion and should be off work the next day.
EE stood up to get another employee attention and forgot the door was open and fell. When EE fell she hit her side on the floor which is what broke her ribs.

CHAIN BROKE HIT INJURED ONLEFT HAND, KNOCKED OFF HARD HAT & SAFETY GLASSES. INJURED WAS STANDING ON CATWALK NORTH SIDE OF IMPACT OPERATOR HOUSE. CHAIN NEAREST THE ROCK WAGON BROKE & WENT BACK OVER FEEDER TO THE NORTH SIDE OF THE IMPACT OPERATOR HOUSE
