Transferring drill pipe to forklift
Making modifications to hydraulic pump when hydraulic fitting failed injecting worker with hydraulic fluid through left forearm.

Pulling Wheel barrow from hoist. Handle broke with sudden release he fell down.
While welding on a four inch pipe sparks contacted flammable material that was released from an adjacent perforated drain pipe.
Two employees were attempting to remove drill steel from shaft jumbo. Employee positioned at top of drill dropped wrap-wrench, contacting injured employee on face. Sutures utilized to close wound.
EE was subject to a pre disposed hernia ailment, confirmed through a medical evaluation obtained on 01/22/2014.
