Employee operating the Crusher saw the belt stop on the machine and got off the machine to shut it down; on EE's way to the shutdown button, EE tripped and hit EE's head on a rock on the ground, cutting head. Received 3 stitches.
Miner was replacing a track pin in a crusher. EE smashed a finger when EE swung the 3rd time with the hammer. Taken to emergency room in Ashland, KS.
Employee was holding the head of a carriage bolt with fingers, another employee was running impact wrench. The impact wrench spun the bolt cutting EE's thumb.
Employee hit door with hammer to loosen door, and struck thumb with hammer in the process of opening door.
