Employee was welding and thought a piece of dirt or something flew behind his face shield and into his eye. He worked the rest of the shift and did not wish to go to the doctor. He worked the following day and after the shift went to doctor on 8/28/14. Doctor found a small a metal shaving in eye.

On 3/1/12 EE stepped in a hole and fell backwards but did not miss any work or go to a doctor at that time. EE worked until 4/19/12. W.C.reviewing

EE was using a chain saw to cut a tree approx 18-20" in diameter. When the tree fell, a grapevine that was unseen in the top of the tree pulled a dead tree approx 6-8" in diameter down and struck EE, knocking him to the ground. When EE hit the ground, he fell on a rock that caused a laceration over right eye, requiring 7 stitches.

EE was climbing out of Ingensol Rand Drill, when the step broke off where it has been welded. EE fell about 4' down and landed on his feet. His back started hurting later on and he went to the doctor on 2/5/07. He was put off work for 1 week for rest.
