Working in cab on D9N, and started down out of the cab. Stepped down on track and twisted left knee. Heard it pop, but thought it would be ok. Did not feel need to go to doctor. On 8/15/14 he was examined and scheduled for MRI, but returned to work. He missed work 8/19/14 due to pain but returned on 8/20/14 and worked through 8/26/14. MRI scheduled for 9/2/14.

EE was getting off 992 loader at end of shift when he said he hung the toe of his boot on the boot brush causing him to fall onto platform. Had lunch bucket in left hand and fell onto left shoulder.

EE went to doctor for pain in left knee. X-rays were negative, but when he told doctor that he worked at a mine, the doctor says that his spine is twisted due to work at the mine and that was the cause of the pain. Pain has now moved to the back, according to the EE. Has follow up appointment on 10/6/09 for MRI.

EE was operating Carerpillar 777D rock truck & left the loader with load of rock & drove over the berm & rolled 1 time down embankment approx 20 ft & landed on its tires. EE said he lost sight of the road in the dust. EE was admi to hospital & tests were run but only possible fracture to L collarbone was found.

Employee was climbing down ladder on 992C endloader when his foot slipped and he hit his right elbow on the handrail. He went to the doctor the next day but x-rays were negative. Elbow kept swelling up and hurting. Was referred to specialist who performed surgery on 01/19/2005 to remove some scar tissue and a bone chip.
