Employee was found outside of the mine laying on the backside of the Slope Belt. His last known work location was around the Main South Belt Tailpiece area. His only memory of the accident is being on top of the Main South Belt and striking the top of his head on a roof bolt. The cause of the accident is unknown.
Injured employee was standing in between two (2) mobile rides, when one (1) of the rides was struck by a scoop, knocking the rides into each other, pinning the employee causing a fracture of the right lower leg.
Employee attempted to catch pin steels as they fell from the drilled hole. In an attempt to regain his balance, his right hand landed on top of the roof bolter pod and his left hand accidentally grabbed the raise lever causing the boom to lift, pinching his little finger between the pod and extension arm.
A roof fall occurred in the 1st West Sub-main, Spad 10+50, entry #9. The fall measured 18' wide X 6' high X 30' long, approximately.
A maintenance trainee was splicing a cable and cut his left thumb with his belt knife, requiring (12) twelve stitches.
Employee was bending a 5ft roof bolt and felt pain in his left testicle/groin.
Employee was installing a 9 ft cable bolt and felt his right shoulder pop.
employee was walking toward the bolter machine. He stepped into a hole twisting his right knee.
A roof fall occurred in the 3rd South Sub-main, between spads # 0+00 & 0+70, entry # 8. The fall measures 20' L x 20' W x 8' H.
Injured employee (rb operator) was inserting his extention steel, a piece of draw slate (10" L X 8" W X 2" T) fell striking the employee's Right Hand, (little finger) causing a laceration, requiring stitches.
A roof fall occurred in the 2nd South Panel @ spad # 24+20, Entry #2. The fall measures 19'L X 19.8' W X 10' H.
Employee stated he arrived at the working section, exited the mantrip walking toward the face and felt pain in his middle and lower back.
Injured employee got his left hand caught between the roof bolter canopy and the mine roof.
A roof occurred in the 1st West sub-main, entry #9, spad # 8+40. The fall measured 25" long X 19' Wide X 8' high.
Injured Employee was struck by a rock measuring 20"l X10"W X 3" thick causing a contusion to the left side of his face.
A rock measuring 45" W x48" L x2" Thick fell (approximately 46") from between roof bolts. Upon impacting the mine floor, the rock rolled inward with the outer edge striking the employee's right ankle.
A roof fall occurred on the 1st West Submain,spad # 14+00, entry #6. The fall measured 19ft W x 19ft L x 6ft H.
A roof fall occurred on the 1st West Sub-mains @ spad # 4 + 20, cross cut #11, between #4 & #5 entries. The fall measured 20'L X 18.6'W X 6'+ H.
A Roof fall occurred in the intersection, last open cross cut, inby the tailpiece on unit #1, Entry #1,of the 1st South Sub-mains. The fall measured approximately 15'L X 18' W X 9' H.
a rock (12"w X 18"l X 3"t) fell from between roof bolts striking the victim on the left hand, little finger.
A roof fall occurred on the 1st West Sub-main at spad # 6+30, entry #6. The fall 18' long X 19' wide X 6' high.
Roof Fall occurred on 1st West Sub-main @ spad 4+90 between 6 & 7 entries. The fall is approximately 20' wide X 20' long X 8' high.
A roof fall( 50 ft long X 19ft wide X 9ft High ) occurred at spad 5+60, 1st West Sub-main, Entry #10.
A roof fall occurred in the 1st West Submain on #1 unit inby spad number 7+00 in the face of #7 entry measuring approximately 50ft. long x 19ft. wide x 8.5ft. high. This area was supported with 5ft. fully grouted roof bolts and 9ft. cable bolts.
A roof fall occurred in the #3 entry, cross cut #21, 3rd South Submain. 25 ft long X 19 ft wide X approximately 5 ft high.
Injured employee stated a rock 12" thick x 72" long X 24" wide fell, stricking his right leg, causing a contusion.
Injured employee was in the process of unloading material from a trailer, for a belt installation. He began having pain in his lower back.
A roof fall occurred in the 3rd South sub-main (#1 unit) #7 entry at spad 0+70 measuring approximately 45ft. long x 25ft. wide x 8ft. high. This area was supported with 5 foot fully grouted roof bolts.
Employee was in the process of installing a roof bolt and was using the boom of the machine to push the pin into the hole when he got his left hand caught between the boom and the pin causing a laceration to his little finger requiring six (6) stitches.
A rock (4' x 3'x 3" thick) slid from atop the loading section of a ram car landing on top of the injured employee's left foot.
Employee was cleaning pieces of roof rock out of the walkway on the 2A belt and caught his middle finger, on his left hand, between two pieces of rock causing a fracture to the finger.
Employee was in the process of splicing a cable when the twisted cable slipped in his hand as he was splitting the jacket of the cable. The knife blade struck the palm of his hand cutting it open in a slicing motion.
A roof fall occurred in the 1st SW Submain in the #8 entry intake escapeway at XC 9+30. This was in a slip area. The fall measured 30' X 30' X 8'. Some 9' cable bolts were in the intersection, but were pulled out or came out with the rock. Visible slips and wet soft bottoms that is present will require more support, such as cribs, timbers and prop-setters.
At approx. 3:40 am, a roof fall occurred in an intersection on No. 2B supply road in the No. 6 entry. The fall was approx 5,800 feet outby the working face. In addition, the fall was approx 30 feet long, 15 feet wide & 9 feet thick. It did not affect ventilation or passage of men. It did not fall above the anchorage of the bolt being used. Additional Cribs & Rails will be installed
The EE was assigned to service the mantrip located outside the mine shop.As he was walking out of the shop, he tripped over the corner of a wooden pallet that was laying on the ground next to the shop. While he was walking out of the shop, he was carrying a quart of oil that he was going to put in the mantrip. He fell forward & landed on his right knee & right shoulder.
EE was in the process of changing a flat tire on a 3-wheeler transport ride underground on #2 unit. He had hooked a chain to the front of the 3-wheeler and was in the process of raising the ride about a foot and a half, when the chain came off, causing the ride to hit him on the left knee. Because of low coal, he was on both knees. He was taken to a local hospital for cut to knee.
Employee was in the process of reclaiming belt structure and was loading the structure in a scoop bucket. While doing so, he accidentally placed his left foot underneath the bucket as it was being lowered to the floor. As the bucket came down, it pressed down on top of the foot and fractured the tarsal bone. NOTE: He was wearing metatarsal boots.
Starting in intersection in last open xc outby 70' to 90', an unplanned roof fall occurred in #1 entry on #1 unit starting at 23+80 to 24+50. There is a slip running through this area. The area has been timbered out and cribbed off and flagged off to keep anyone from entering. Fall was 14' to 18' thick and 8' high.
EE stated that he was in the process of roof bolting the face of the #2 entry on the #1 unit. While pulling the drill steels back out of the roof, one of the drill steel sections stayed in the mine roof. While the ee was placing drill steel sections in the roof bolter's tray, the steel that had stayed in the roof fell out striking the ee's finger on his left hand.
Employee was in the process of reclaiming a water line at the old #2 beltline. As he was lifting the water line up to a trailer, his left ear scrapped against a belt chain that was hanging down from the mine roof causing a small cut to the inner portion of the ear. Later that evening, he went to the medical clinic and received 5 stitches.
The employee stated that while he was roof bolting, a piece of roof rock fell out striking the head of the roof bolting machine and bounced off hitting him in the mouth cutting his lower lip. First aid was administered and he completed his shift. Later on, he decided to go to the emergency room. While at the ER, he received 5 stitches to the lower lip.
Employee had just completed greasing fittings on the back of the roof bolting machine. When he got up and turned around to walk away from the bolter, a piece of loose rock fell striking him on the left lower arm and hand. The ee was using a walking stick and rock hit his hand driving in down on the walking stick. He has a small fracture behind the 2nd knuckle on his left hand.
EE stated that while operating a ramcar on the section, he dipped down in a hole & then bounced up throwing him against the roof of the canopy. The impact caused him to hit his head against the canopy. He was semi-unconscious immediately afterwards & then responsive. Taken to the hospital, treated & released. On 7/28/08 he went to a chiropractor, & now has begun missing work.
The EE was sitting down at the NO.1 unit battery shack, preparing to change a 4-man ride tire. While doing so, he started having shortness of breath and then said that he was experienching chest pains. The pain was in the center of his chest, with tingling in his left hand. He told 2 co-workers he wanted to sit down. A few minutes later he said he was having a heart attack.
The injured employee was in the process of hanging the left side roof bolter cable that was lying on the mine floor to make sure that no one would run over it. As he picked it up, he received an electrical shock to the right hand. He was examined and placed under observation at the hospital. He returned to work the following Monday.
While moving across the unit, a piece of rock (3'x1'x1"thick) fell out between pins and struck the EE on the back and left hand. He returned to work the next 2 days and then began missing work.
A roof fall occurred in the morning of 5/6/08 on #1 unit in the #4 entry approx 70' long, 18' wide and 5' high. During investigation it was found that a kidney bed had been encountered in the one entry only.
While moving belt structure underground, the employee stated that he twisted his left knee. He has not missed any work up to 5/28/2008. According to a doctor who recently examined him, the employee has a torn meniscus in his knee. He is scheduled to have outpatient surgery on 5/29/2008.
A roof fall occurred between the afternoon of 10/8/07 and the morning 10/9/07 (mine on idle status) one shift per day five miners keeping the mine rehab. 1st sourth panel-no. 1 entry 4 x cuts from face of no. 1 unit 120 ft long 19 ft wide and 5 ft high. spad no 6+08.
A rock fall occurred on #2 unit at 5+10 in the 1st south panel. The fall was approximately 80 x 19 x 6. Unit was moved to next set of intake rooms.
A rock fall occurred at 12+60 in the 3rd east submain in entries 5 & 6. The fall was 90 x 19 x 6 and ran north & south. The fall will be cleaned up.
A rockfall occurred @ 1pm on the #2 Unit in the 3rd West Panel. The fall blocked our secondary escapeway. The fall was between #3 & #4 entries, extending to #5 entry. The fall was 10x19x9. Msha was notified within 15 minutes.
A rockfall occurred on the 2C belt. The fall was 40 x 19 x 5. Five foot glue bolts were used in this area. The area was advanced on 70 x 50s. There was a hidden slip - no water in area.
Employee stated pinner steels were stuck and he was trying to pull apart. When they came apart struck him in lower lip.
Employee was standing & starting complaining of knee pain. He's a brittle diabetic with many aches & pains in his joints. After medical exam it was determined he was suffering from diabetic neuropathy.
Employee was getting ready to move miner when a piece of rock fell striking left shoulder and arm. He was on #2 unit in #1 entry cutting 1 left.
#2 Unit had a rockfall inby the dumping point in the #5 room at 2+85 to 3+20. The face was 120 feet inby the crosscut. There was a slip. The fall was in the pillar line.
#2 unit had a rockfall inby the dumping point in the intersection of #8 room at 2+75. The face was 15 feet inby the crosscut. There was a slip.
He had bone spurs upon exam.
A roof fall occurred at 15+60 in the 4th south panel. The fall was 80 x 19 x 10. The area had 5' glue bolts & 12' cable bolts in it. The belt only had 5' glue bolts. The fall was 3800' outby the working faces. Fall will be cleaned up.
A rock fall occurred ap[prox. at 5:00 am on #1 unit in the intake from #8 entry to #6 entry blocking the intake. Some intake was open in #8 entry from rib approx 5 feet. Rockfall was 180 X 6 X 19 feet, 40 feet outby working faces.
A rock fall occurred on #1 unti -#8 entry in the intake. Fall was from 17+55-18+65 a total of 110 ft. fall started outby feeder and extended 55 ft inby-110 ft from face.
Employee stated he was beside pinner in the #6 entry on #2 unit when a piece of rock fell striking rt leg between knee & ankle. The rock caused 2 fractures (1 6 inches above ankle on right side of leg & 1 3 inches above ankle on left side of leg.) The rock was 2.9 feet wide x 4.8 feet long x 6 inches to 1 inch thick.
A rock fall occurred at #6 entry #1 unit fro 8+50 to 10+00. The unit has moved out of the panel. MSHA district office was notified.
An unintentional rock fall occurred on #1 unit #3 entry inby 3+50 to inby 4+50. This is a neutral entry - the face is at 11+00. The fall will not be cleaned up at this time.
A rock fall occurred at 13+50 on the #2 unit. This was 1800' outby working section. Fall was from edge #5 entry across panel into old rooms - Return side rooms - the unit was idled until fall was cleaned up in supply road and return.
Examiner found rock fall in return main south-# entry @ 66+40. Must have benn hidden slip-no water. Fall was 6350 ft outby dumping pt. on unit #1.
A roof fall occurred on #1 unit-#8 entry in the intake at plus 32 & 20. The fall was in an section and extended both direction 15 ft. the fall was outby the working section 850 ft. the fall was 18x 51/2 x 50.
EE was splicing a blown waterhose when the band broke off in the banding machine. He got another bander and finished fixing the waterhose. He took out his channelocks and tried to pull the broken piece of band out of the machine. When the piece came out it struck his right thumb causing a laceration that needed 8 sutures. He had removed his gloves because they were wet & slippery.
A ROCKFALL OCCRRED IN THE INTAKE ON #2 UNIT-THE FALL WAS FOUND BY EXAMINER. 20X18X51/2
A ROCKFALL OCCURRED SOMETIME OVER THE WEEKEND ON #1 UNIT. THE FALL WAS IN #3 ENTRY-NEUTRAL BETWEEN 28+30=29+10. 80X18X51/2
Examiner found rockfall in #8 entry - #1 unit in the intake - The fall was 140 x 18 x 5 1/2 - outby the face about 1300 ft.
A rock fall occurred on #1 unit, #8 entry intake. The fall was 80 x 18 x 5 1/2.
Employee was walking in old belt entry and his left ear hit a belt hanger plate cutting ear - stitches were received.
Emplyoee stated he had to help partner unstop suction hose and was getting up when he raised boom catching pinky in pinch point.
ROCKFALL # 1 UNIT - #5 ENTRY-BELT - FELL ON FEEDER- INBY 50 FT. FALL WAS 170 FT OUTBY FACE. FELL EDGE OF FEEDER
ROCK FALL WAS FOUND IN THE INTAKE ON #1 UNIT IN ENTRY #6 BETWEEN 14+40 & 15+40. FALL WAS 100' X 19' X 5.5'.
EMPLOYEE STATED HE HAD BEEN OPERATING RAM CAR ALL SHIFT & COULDN'T REMEMBER DOING ANYTHING EXCEPT MOVING ONE TIMBER THAT HAD FALLEN INTO THE ROADWAY. HE HAS HAD (3) PRIOR BACK SURGERIES & ALSO HAS DEGENERATIVE DISC DISEASE, WHICH IS PROGRESSING EACH TIME HE GOES TO HIS SPECIALIST.
Found rock fall in #1 entry on #1 unit return between 11+90 & 12+60. Do not know when it fell. No water coming in.
A ROOF FALL OCCURRED ON #2 VN.T. THE FALL WAS 60 X 19 X 4. THE FALL WAS IN #3 ROOM @ 2+50. IT WENT THROUGH XCUT TOWARD #4 & TOWARD #2 ROOMS.
Rockfall found in return #1 unit - entry #3 return No idea when rockfall occurred 20 by 18 by 6 size.
AN UNINTENTIONAL ROCK FALL OCCURRED OUTBY #1 UNIT IN THE MAIN SOUTH IN #3 ENTRY AT 98+40 TO 99+60. THE FALL WAS 120 X 19 X 6 - #3 ENTRY IS A NEUTRAL.
EE twisted knee 1-10-05 after going to Dr and not missing any work it was decided to do some cleaning of knee. Diagnosis was osteoarthritis of knee, but an MRI showed tear which was repaired on 3-3-05 while knee was being checked. EE's first day to miss was 3-3-05 original injury was 1-10-05.
ROCK FALL OCCURRED IN #5 ROOM 1+20 FT. OUTBY WORKING FACE, 300 FT. INBY DUMPING POINT. NO WATER OR ADVERSE CONDITIONS COULD BE SEEN.
Roof fall occurred on the #1 unit in entry #3 a neutral- the fall was 20 by 20 by 5 1/2. The unit had gone through a slip earlier in the week. 12 ft cable bolts had been installed in other entries.
A rock fall was found at 4 am-4:15 am by the fireboss. the fall was on #1 unit int he intake at 10 & 90 and went from #7 entry to #9 entry. the area had cribs built and 12' cable bolts in the intersection of #8 entry. the fall will be cleaned up thru intersection 7 & 8.
UNIT WAS TRAVELING UNDER CREEK AND RAN INTO SOME BAD TOP. UNIT WAS PUTTING CABLE BOLTS IN XCUTS BUT HAD GOTTOEN TO #6 AND A ROCK FALL OCCURRED. IT WAS 40 X 19 X 6 AND WENT FROM #6 ENTRY TO #5 ENTRY THROUGH THE XCUT AND STOPPED 20' INTO THE XCUT.
IN INTERESCTION 10 + 20 ON #1 UNIT IN ENTRY #2 RETURN SIDE WATER WAS DRIPPING AND INTERSECTION FELL IN - 5 + 6. PINS WERE USED IN THIS ENTRY. WATER IS STILL DRIPPING IN INTERSECTION. FALL WAS 40X35X6.
Employee was loosening bolts and when they loosened the ratchet let his hand strike a sharp edge cutting the right pinkie (five stitches).
FOUND ROCK FALL IN #2 UNIT INTAKE IN #8 ENTRY FROM 10 FT OUTBY 19+90 TO 21+20 PLUS. DO NOT KNOW WHEN IT FELL. NO WATER COMING IN BUT PULLED 6' RESIN BOLTS. FOUND WHILE DOING ROCK DUST SURVEY. 130 X 19 X 5 1/2.
Ee stated back had been hurting for about a week after rock fhit in shoulder. took to hospital having muscle spasm did not miss any work with back until 7-13-04-went back to dr with muscle spams and taking physical therapy.
A rock fall occurred when roof became saturated with water, area had been rail barred and also had been cribbed and 12 ft cable bolts installed. We were in process of removing loose top with miner in belt when it occurred.
EE AND THREE OTHERS WERE HANGING HI-LINE WHEN HE STATES HE FELT PAIN IN LOWER BACK.
ROOF FALL OCCURRED IN THE #2 ENTRY RETURN MAIN SOUTH AT 66+40. THE FALL WAS IN THE INTERSECTION. 6 FT. REBAR (TENSION BOLT).
ROCK FALL OCCURRED IN THE #1 ENTRY ON THE #1 UNIT 60 FT OUTBY THE DUMPING PT WHICH WAS BETWEEN 3 +50 & 4+50 IN THE RETURN.

Employee was setting 6 by 6 timbers in old works-return and felt pain in rt groin area (hernia) worked until 4-7-04, surgery 4-8-04.
THE MINER HAD FINISHED CUTTING THE FACE OF #2 ENTRY ON THE #1 UNIT WHEN THE CUT FELL AND PULLED 3 ROWS OF ALREADY BOLTED ROOF.
AN UNKNOWN SLIP WAS INT HE #6 ENTRY ON #1 UNIT IN THE 4TH EAST PANEL INBY THE FEEDER 100 FT. THE FALL WAS 20X19X7 CRIBS HAD BEEN SET AND BOLTS SPOTTED.
THE AREA WHERE THE FALL OCCURRED HAD A SLIP IN THE INTERSECTION WHEN IT WAS CUT THE AREA THAT FELL WAS ACTIVALLY A CONTINUATION OF THE SLIP. THE AREA WAS AT 2 & 70 IN THE 4TH EAST PANEL IN THE #1 ENTRY ON UNIT #1. THE AREA WAS AT AN INTERSECTION AND WAS 20 X 19 X 6-7 FT HIGH.
EE WAS STRAIGHTENING A 5 FT BOLT TO INSTALL WHEN HE FELT PAIN IN HIS LEFT WRIST.
EE WAS STANDING BY BOLTER INBY LAST OPEN X-CUT WHEN A PIECE OF ROCK FELL STRIKING LOWER LEG & ANKLE, A HAIRLINE FRACTURE WAS FOUND IN LOWER LEG & TENDONS & LIGAMENTS PULLED IN RT. ANKLE.
EE WAS DROPPING BOOM DOWN AND PICKING UP ROOF BOLT MASHED FINGER BETWEEN BOOM AND ROOF BOLT. EE NEEDED 3 STITCHES IN LEFT RING FINGER.
ROCK FALL ON #2 UNIT SUPPLY ROAD. APPROXIMATELY 1600' OUTBY UNIT BETWEEN XCUT 12+20 & 13+90 ON 2ND EAST PANEL.
IN THE #8 ENTRY OF THE 2ND EAST PANEL A ROCK FALL WAS FOUND WHEN THE FIREBOSS WAS MAKING HIS WEEKLY EXAM. THE FALL WAS 70' LONG, 18' WIDE, 5' HIGH.
INTAKE BETWEEN #7 AND #8 ENTRY, X CUT #8 ENTRY WAS DROPPED AND FALL CRIBBED OUT #1 UNIT. 30 FT LONG 18FT WIDE, PULLED 5 FT BOLTS.
WHILE LOADING BOX OF GLUE ON ROOFBOLTER HE FELT PAIN IN LOWER BACK.
EE HAD JUST STARTED O.T. SHIFT WHICH WAS 2ND SHIFT WHEN HE PICKED UP A 50 LB BAG OF ROCK DUST - HE HELT PAIN IN HIS LOWER BACK - HE WORKED THE FOLLOWING DAY SHIFT BUT HASN'T WORKED SINCE.
EE WAS HANGING THE MINER CABLE IN #8 ENTRY. HE WAS ON HIS KNEES PUSHING UP THE CABLE SO IT COULD BE TIED TO ROOF BOLTS WHEN HE FELT PAIN IN HIS LOWER BACK AND LEFT HIP.
IN THE RETURN OF 2ND EAST PANEL, WHILE WITH MSHA INSPECTOR, A ROCK FALL WAS FOUND IN THE RETURN,THIS FALL WAS LOCATED IN THE #2 UNIT RETURN. THE FALL WAS 5'HIGH X 18'WIDE X 150'LONG, AT 9+40.
EE GOT OFF RAM CAR TO USE THE BATHROOM AND UPON GETTING BACK IN RAM CAR STRUCK FOREHEAD ON CANOPY.
IN PANEL 1 - ROOF FALL 40' LONG X 18' WIDE X 6' HIGH FOUND WHILE CHECKING AIR COURSES FOR WEEKLY EXAM. A SECOND SHIFT WORKER NOTICED AT APPROX. 9:00 P.M. A ROOF FALL ON #1 UNIT, OUTBY APPROX. 770' IN THE INTAKE OF #6 ENTRY.
PINMAN WAS STARTING 1ST PIN, WHILE DRILLING FIRST HOLE, JACK BLOWED, CAUSING CANOPY TO STRIKE HIM ON THE HEAD. IT JARRED HIS NECK AND SHOULDER AREA.
EMPLOYEE SAID "HE WAS BENDING A ROOF BOLT WHEN HE PULLED A MUSCLE IN HIS BACK."
HE WAS TRYING TO PUT ANOTHER STEEL IN LINE IN DRILL HOLE. IN DOING SO HE HIT THE WRONG LEVER SMASHING LEFT THUMB BETWEEN BOLTER CANOPY & STEEL IN HOLE.
ROOF FALL ON #1 UNIT IN #1 PANEL AT SPAD #7+70 IN #5 ENTRY.
TONY WAS DRILLING HOLE IN ROOF, PULLED STEEL OUT& TOP STEEL HUNG IN HOLE, HE REACHED OVER TO CHANGE STEELS WHEN A STEEL CAME OUT HITTING HIM ON TOP OF MIDDLE FINGER. FINGER WAS BETWEEN
EMPLOYEE WAS OPERATING ROOF BOLTER. HE WAS TIGHTENING A BOLT WHEN THE WRENCH SPUN OUT OF THE CHUCK STRIKING HIM IN THE MOUTH AND BREAKING A TOOTH.
EE WAS IN PROCESS OF SWINGING BOOM OF BOLTER WHEN THE FOOT JACK SLIPPED OFF OF A PIECE OF ROCK CAUSING THE CANOPY TO COME DOWN HITTING EE ON THE TOP OF HIS HEAD
EE WAS PUTTING UP A ROOF BOLT WHEN A PIECE COAL FROM THE ROOF POPPED OFF AND GOT IN HIS EYE. (RIGHT) HE DID HAVE HIS SAFETY GLASSES ON.
EE EXPERIENCED PAIN IN HIS BACK AND SIDE WHILE TRYING TO GET OUT OF THE SCOOP.
