The task was to connect the rigging to from the trackhoe bucket to the payloader. As the employee was connecting said rigging, the trackhoe bucket contacted the employees left shoulder and face.

Employee claims that he injured his upper left back, left hand, right elbow and right shoulder while shoveling material.

Employee drove front end loader over a pot hole, throwing him against the windshield. Employee was not properly wearing his seat belt.

Employee complained that he had something in his eye after he had left work. He had been grinding metal earlier in his shift and may have gotten a metal particle in his eye when he wiped his face with his sleeve.
Employee tripped over boot cleaner outside of front door to the office and twisted her ankle.
Employee was opening one latch on a camlock pipe cap from a pressurized vessel. After releasing the one latch employee observed that the tank still had pressure. He closed a valve above the cap and then removed the other latch. The line blew the cap off striking employee in the right forearm. Diagnosis, contusion right forearm and corneal abrasion to right eye from dust.
EE was utilizing a pry bar through an inspection hole on a rock hopper trying to free a jammed rock. When the rock came free, the rock fell against the pry bar in the inspection hole. EE still had his hands around the pry bar. EE's finger was lacerated when the pry bar moved up and into a steel support for the hopper.

Employee was climbing down from cab of Volvo L70E front end loader. He had both hands on grab rails of ladder, his right foot was on bottom step, he lowered his left foot onto the ground. Upon placing weight on his left foot, employee said his knee twisted causing pain. Diagnosis from medical doctor describes injury as hamstring muscle strain.
UNKNOWN. EE MENTIONED TO FELLOW EE OF AN EYE IRITATION PRIOR TO LEAVING WORK AT 5 PM. THE FOLLOWING MORNING THE EE WENT TO CLINIC TO HAVE EYE LOOKED AT. FORIEGN OBJECT REMOVED FROM RIGHT EYE. REPORTED ACCIDENT AT APPROX. NOON TO MANAGEMENT ON 3-3-4.

WHILE INSPECTING PIPE MANIFOLD IN TANK. EE CUT PALM OF HAND ON 1" PIPE FITTING. FITTING WAS WORN EXCESSIVELY FROM ABRASION, SHARP EDGES COULD NOT BE SEEN. HIS LEFT PALM REQUIRED STITCHES. HE R ETURNED TO WORK AT FULL CAPACITY.
WHILE ATTEMPTING TO CLOSE DOME LID LATCHES ON LID OF TRAILOR, EE KICKED THE LATCH WITH THE HEEL OF HIS BOOT. HE MISSED THE LATCH & HIS ANKKLE MISSED THE LATCH MECHANISM. DR. CONCLUDED SPRAINED LEFT ANKLE. EXPECTED RELEASE TO RETURN TO WORK ON 4/20/98

EE UNLATCHED CAMLOCK ON HOSE WITH 20 PSI HOSE. HOSE BLEW BAKC & HIT HIM IN THE LOWER ABDOMEN. TAKEN TO HOSPITAL ER, STAYED NIGHT FOR OBSERVATION & RELEASED THE NEXT MORNING.
