Employee was in the process of bagging 88lb bags of barite. Employee removed a bag from the horn and as he turned to stack the bag on a pallet, he felt a pain in his lower back. First aid measures were implemented and the employee went home for the day. Employee continued to work his normal shift the next days.

The employee was in the process of servicing a new piece of equipment (Loader). While sitting on the platform of the fender on the equipment, the employee slipped off the fender to the ground. The employee landed on the ground feet first twisting his right ankle.

Employee noticed that mill feed tank had overflowed. As he ran to shut off the conveyor his hard hat fell off. A rock fell from the conveyor and struck him on the head.
Employee complained of numbness in his right arm after bagging barite in 88 lb. bags. This was not his normal work task. He was removed from this activity and treated with first aid measures. However on 9/10/2009 he complained of continued pain and asked to see a doctor.

Strained back while pulling and twisting to stretch belt for splicing.
Employee had trouble releasing a latch on a self-dumping hopper. He stood on the latch handle and slipped off, falling backward onto the floor, striking his head and back on the concrete pavement.
Employee slipped and fell on a wet concrete surface, landing on his left hip. On 1/16 He reported "a knot about the size of a quarter" to the left hip area and tenderness to the touch. He continued to work while self medicating. On 1/21 he reported continued discomfort and that the knot had not subsided. At that time, he went to see a physician.
