Miner was exiting the water truck. Stepped on rock on the floor which rolled. This slightly tore a ligament in ankle.

Miner was cleaning a tramp magnet. He pulled a piece of steel down and it pulled the piece of steel back up pinching the tip of his index finger.

The miner was installing a rubber screen. It was too long so he decided to trim off the excess with a utility knife. It slid and caused a laceration to his left forearm approximately 2 1/2"" in length.

EE was using a bar to turn a ratchet wrench, pulling toward himself. The bar slipped and hit the EE on the right side of his head.

EE was working on a loader that had its cab & transmission removed. He was where the transmission had been, cleaning the area. He raised an access panel on the hood to give himself more room. His hard hat fell off. He stood to replace it, the wind blew the access panel shut striking him in the head before he could replace his hard hat. He required 4 stitches, 1 day off work.

EMPLOYEE WAS DRIVING A MINE HAUL TRUCK. HE MADE A WRONG TURN IN THE MINE, AND WHEN HE DISCOVERED THE WRONG TURN HE BRAKED WHICH THEN CAUSED THE TRUCK TO SLIDE IN THE MUD SLAMMING INTO THE MINE WALL.
