THE FALL WAS FOUND ON A FIRE BOSS RUN. A HORSEBACK ACROSS IN ENTRY HAD BECOME WET A WEEKEND THE TOP. A 10'X20' PIECE FELL ACROSS THE INTERSECTION 26 BREAKS UNDERGROUND.
THE FALL WAS FOUND AT THE BEGINNING OF THE SHIFT. A HORSEBOOK IN THE TOP COME OUT WITH NUMEROUS KETTLEBOTTOMS. THE AREA WAS CRIBBED & DANGERED OFF.
THE #1 ENTRY INTAK IS CLOSER TO THE OUTCRTOP & HAS NUMEROUS ROOF CHANGES FROM SHALE TO SANDSTONE FROM THE 25TH BREAK TO 35TH BREAK. MOISTURE IN THE TOP WEAKENS IT. VENTILATION NO TRAVELWAY IS NOT ALTERED.
TWO FALLS WERE FOUND WHILE TRAVELING THE INTAKE AT BREAK 29 + 39. THE INTERSECTION HAD BECOME WET, THE AREA AT EACH FALL WAS CRIBBED. VENTILATION OR TRAVELWAYS WERE NOT INTERUPTED.
THE NEARBY AREA WAS ALREADY CRIBBED. EXCESSIVE RAILFALL MIGHT HAVE CAUSED THE ROOF TO TAKE ON MORE WATER, CAUSING THE FALL.

EE WAS PREFORMING CLEAN UP WORK WHEN A CHUNK OF ROOF FELL FROM BETWEEN THE BOLTS. THE ROOF LANDED ON HIS SHOULDER AND BACK.
THE ROOF WAS SOFT AND WATER DRIPPING FROM TEST HOLES COAL STREAKS HAD BEEN FOUND IN THE AREA. THE INTERSECTION IN 2 ENTRY BEGAN WORKING AT 800 AM THE AREA WAS CRIBBED BUT THE INTERSECTION FELL AT 1030 AM.

THE ROOF BOLTER OPERATOR INSTALLED THE FIRST TWO BOLTS AND STARTED TO ADVANCE TO THE NEXT ROW. A PIECE OF DRAW ROCK 3" THICK FELL AND RODE HIM DOWN. THE CANOPY HAD BROKE THE ROCK, CAUSING IT T O FALL FROM THE BOLTS TO THE RIB. 7' X 3.5'
HE WAS INSTALLING A BUMPER RAIL ON MINER & DROPPED IT ON HIS RIGHT HAND. DID NOT GO TO DOCTOR UNTIL 11-21-95 BECAUSE HE THOUGHT IT WAS OK.
HEW WAS OPERATING CONT. MINER WHEN THE CAR DRIVER HIT HIM. INJURED HIS KNEE.
THE AREA OF THE FALL WAS WET & CONTRIBUTED THE FALL. THE TEST HOLES WERE SCOPE & SHOWED AT CRACK AROUND THE BROW OF 40" INCHES. THE INTAKE TRAVELWAY WAS REROUTED.
EE WAS WELDING OF PIECE OF FLUX WHEN A SPARK FROM THE FLUX FLEW INTO HIS EAR AT THE TIME IT HAPPENED IT DID NOT BOTHER HIM BUT ON MONDAY 09-18-95 IT STARTED HURTING & HE WENT TO THE DOCTOR ON TUESDAY 09-19-95
HE LIFTED A ROCK AT THE TAILPIECE OF THE TRACTOR AND HURT HIS BACK.
HE WAS OPERATING ROOF BOLTER WHEN BOLTER WRENCH SLIPPED OFF AND HIT HIM ON THE LEFT EYE.
THE NO 1 ENTRY IN #2 SET OF ROOMS LEFT OFF #2 MAINS BEGAN WORKING THE AREAS WAS CRIBBED OFF & FELL BETWEEN THE CRIBS SOFT TOP IN OUTCROP AREA VENTILATION WAS NOT INVOLVED
WAS RUNNING THE TRACTOR WHEN RAN OVER A ROCK. HE LEANED OUT TO PICK UP THE ROCK & THROW IT OUT OF THE WAY, WHEN TRACTOR JUMPED & CAUGHT HIS FINGER BETWEEN TIRE & ROCK.
EE STANDING TO CLOSE TO FIRE BARREL DUE TO COLD WEATHER CAUGHT HIS COVERALL ON FIRE BURNING HIS LEGS.
MAKING BELT SPLICE HAMMERING NAILS WHEN ONE FLEW BACK AND HIT HIM IN THE LEFT EYE.
THE ROOFF BEGAN MAKING WATER NEAR A PREVIOUS FALL AND THE INTERSECTION FELLIN THE RETURN AREA NEED BRATTICE 40. THE FALL IS IN THE RETURN AND RETRICTED AIR.

EMPLOYEE SAID HE FELL ON CONVEYOR BELT WHILE SHOVELING AND CLEANING UP BELT ENTRY CAUSING HIM TO INJURE HIS NECK.
THE FALL WAS A CONTINUOUS FALL OF A PREVIOUS FALL ON THE BELT ENTRY. IT WAS ABOUT 12FT LONG & 8 FT WIDE. THE AREA WAS RECRIBBED & HEADEVED.
EMPLOYEE SAID CRAWLING CAUSED FLUID TO FORM ON HIS KNEES.
EE WAS SUPPLYING BOLTER WITH BOLTS AND RESIN AND CLAIMED HE HURT HIS BACK WHEN HE PICKED UP A BOX OF RESIN THAT WEIGHTED APPROX 40 LBS.
HE WAS OPERATING A ROOF BOLTER WHEN A SMALL PIECE OF SCALE FELL AND HIT HIM ON THE BACK.
EE WAS OPERATING ROOF BOLTER AND STUCK HIS FINGER SOMEWHERE ON THE BOOM AND IT WAS SEVERED TO FIRST JOINT.
THE INVESTIGATION SHOWED WATER OR MOISTURE WITHIN THE LAYERED ROCK, CAUSING THE INTERSECTION TO BE WEAKED. THE ENTIRE AREA WS CRIBBED AND SECURED.
THE ROOF FALL HAPPENED IN NO.6 ENTRY OF THE RETURN NEAR SPAD 175 THE FALL IS ABOUT 60FT X 20 FT 4FT. WATER IN LAMENATED TOP WITH COAL STREAKS CONTRIBUTED TO THE FALL. THE AREA IS TO BE CRIBBED OR TIMBERED.
THE FALL WAS DISCOVERED DURING A PRESHIFT RUN IN A WORKED OUT AREA IN THE RETURN. OUTCROP MINING CONDITIONS WAS THE CONTRIBUTING FACTOR FOR FALLS. NO OTHER DAMAGE WAS SEEN.
THE ROOF FALL WAS DISCOVERED ON A FIREBOSS RUN. SLIGHT WATER HAD BEEN NOTICED PREVIOUSLY AND CONTRIBUTED TO THE FALL. SOFT TOP IN THE OUTCROP ROOMS ALSO CONTRIBUTED.
EE WAS RIDING MANTRIP INTO MINE TO START TO WORK. AS HE WAS GETTING OUT OF MANTRIP HIS JACKET GOT CAUGHT ON ROOF BOLT AND WAS DRAGGED OUT OF MANTRIP.
THE ACCIDENT OCCURRED 4 BREAKS INSIDE IN AN INTERSECTION O NO. 6 ENTRY. AN UNKNOWN HORSEBACK (KETTLE BOTTOM) BROKE LOOSE AT 4' TO 5' THICK. IT PARTIALLY BLOCKED ONE INTERSECTION IN THE RETURN.

HE HAD FINISHED WELDING ON A PIECE OF EQUIPMENT. HE JUMPED OFF THE EQUIPMENT INJURING HIS KNEE. COMPANY WAS NOT PRODUCING COAL YET. THIS IS A NEW MINE.
