THE FALL WAS IN THE INTAKE- #6 ENTRY, 15 BREAKS INSIDE SOFT SHELL TOP 4' BOLT & MOISTURE IN THE TOP CONTRIBUTED TO THE FALL. NO ONE WAS INJURED & VENTILATION WAS NOT AFFECTED. THE AREA WAS CRI BBED & ROADWAY WAS MOVED.
