Employee was trying to "stand up" a rear tire from a 1987 Ford service truck, which was lying flat on the ground. Employee knelt down and took hold of tire, then attempted to stand tire up and as he did, he claims to have heard a "snap" in his back.

Incident reported on 5/2/05, stating that lower back started hurting & pain increased thru the week. A new seat was ordered & installed. Restrictions issued by doctor on 5/2/05 did not inhibit him from performing his regular job duties as a plant operator but our company safety director contacted MSHA in Colorado on 5/19/05, and it was determined to be reportable to MSHA.

EE WENT TO STEP ONTO WET STEPS AND HER FOOT SLIPPED. SHE FELL ON HER RIGHT KNEE AND CUT IT AS WELL AS SWELLING THAT STARTED TO OCCUR. SHE WENT TO THE CLINIC THAT DAY AND WAS PUT ON RESTRICTED DUTY.

EE DENIED M EDICAL TREATMENT AT THE TIME OF THIS INCIDENT & RETURNED TO HIS REGULAR JOB IMMEDIATELY. EE SOUGHT MEDICAL TREATMENT ON HIS OWN AT A LATER DATE & IT NOW REQUIRES SURGERY, SCHEDULED FOR 3-16-0
EMPLOYEE DIDN'T REPORT INCIDENT UNTIL 8-10 OR 8-11 WHILE CLAIMING HE INJURED BACK 7-29 WHILE WORKING IN LOADER.
