American Aggregates of Michigan Inc · Struck against stationary object

Miner was dislodging accumulated material in a log washer with a ½" steel rod measuring approximately 8ft in length, which was inserted through an opening in the expanded metal cover of the feed head box on the log washer. When the material broke free the rod went further down the chute and caused the employees' left hand to strike the metal cover.

Employee was working on breaking bolts free on the waste pump to inspect inner components. When the second bolt and nut broke free it jarred employee forward causing a slight pop and pain in right shoulder. Employee was using an adjustable wrench for the job because employee did not have a big enough wrench/socket for the job.

Swinging a 4# sledge hammer with both hands to free up a cut piece of metal from beam. Metal broke free and upon completion of swing the left hand middle finger contacted the metal beam, resulting in a fracture of the middle finger on the left hand. Employee is right hand dominant.

Employee was operating a D8 Dozer pushing up material, spun his tracks, this caused and buildup of material behind the tracks. As the employee backed up over this material the rear of the dozer slammed to the ground once it broke the center of the pile. The employee was twisted the right, looking back as this happen, jarring his back.

While working in tight quarters on the crusher, employee hit his right elbow on the frame of the crusher causing it to bruise. Employee was taken to St. Joseph Mercy Maple Health Building where his right arm was put in a sling and he was given a prescription for Motrin.
