EE had carpal tunnel surgery 15 years ago, EE mentioned having symptoms again in Aug. 2011 EE took a LOA starting 9/29/11 and RTW on 4/16/12. After working four days EE informed us that he had seen a Doctor because he has carpal tunnel.

Employee states ""Tripped on door sill to garage - twisted right knee"". His knee continued to bother him so he went to the doctor and now has restrictions - limit prolonged standing, no squatting, crawling or working on knees: prescription for physical therapy.

EE arrived at work 45 minutes early. He was walking into the scale office when he stepped on some ice on the north side of the building. EE slipped and fell landing on his right arm.

WHILE WORKING AROUND GARDEN PLANT IT WAS REAL WINDY AND DUSTY SOMETHING BLEW IN HIS EYE HE WASHED IT OUT AND CONTINUED TO WORK WITH SOME DISCOMFORT. ON 4-27-03 EYE WAS INFECTED SO TREATED WITH PRESCRIPTION MEDICATION. NO LOST TIME.
