Audiometric testing on 9/11/13 indicated EE has experienced a standard threshold shift.

EE had carpal tunnel surgery 15 years ago, EE mentioned having symptoms again in Aug. 2011 EE took a LOA starting 9/29/11 and RTW on 4/16/12. After working four days EE informed us that he had seen a Doctor because he has carpal tunnel.

Employee states "Tripped on door sill to garage - twisted right knee". His knee continued to bother him so he went to the doctor and now has restrictions - limit prolonged standing, no squatting, crawling or working on knees: prescription for physical therapy.
EE arrived at work 45 minutes early. He was walking into the scale office when he stepped on some ice on the north side of the building. EE slipped and fell landing on his right arm.

Audioligist called with audiogram results. Reported that EE has been n edge of STS for years and now has crossed the line.

Employee states that while getting off loader, he missed the last step and twisted something in his back. Employee did not fall, but came down hard on the ground.
EE operates Cat 980G loader 8hrs/day, 5 days/week, for approx. 35 wks/year. His exposure to noise is well below the action level.
EE operates skid-steer loader and performs general clean-up and maintenance. Ee uses muff-type PPE 100% of the time. EE's exposure exceeds action level but not PEL.

WHILE LIFTING A RAILROAD TIE TO USE AS BLOCKING, EE FELT SOMETHING IN LOWER BACK ON 3/23/99. HE DECIDED TO SEE A DOCTOR WHO THEN TOOK HIM OUT OF WORK FOR TREATMENT UNTIL AT LEAST 4/8/99, NEXT APPOINTMENT.
SMASHED RIGHT BIG TOE GETTING OUT OF SKID STEER BETWEEN KICK PLATE AND HOIST ARM WHEN HOIST CAME DOWN. OFF WORK UNTIL DOCTOR APPOINTMENT 3-18-99.
