Employee was using a bar to remove a rock that had become jammed in the Kodiak K400 Cone Crusher. The employee hit the rock with the bar; the bar came back, striking the employee in the face, resulting in a split upper lip on the left side of EE's face. Employee received stitches and was placed on restricted duty.

Employee was replacing 2 v-belts on a conveyor head pulley. As EE was turning the pulley to guide the 2nd belt on, the belt pinched EE's finger against the pulley.
