Employee was changing the center deck screens on the Diester. While pulling the new screen into place the employee heard a pop in left elbow. Employee went to the doctor on 4/28/2022, X-Ray revealed no fracture or chip. No restrictions given. Employee went back to the doctor on 5/16/2022 for an MRI where employee was diagnosed with a torn bicep making this MSHA Reportable.

The employee was getting off a scissor lift using three points of contact when he reached the final step to the ground he stepped on a two inch rock straining his left ankle.

Ee did not dismount the screen conveyor properly using three-points of contact. The ee jumped approx 4 to 5 feet to the ground causing a contusion to his left foot (heel). The ee working with him did use 3-points of contact and was fine. The ee was given a written warning. Prior to that morning a safety talk was given on 3 pt contact.
EMPLOYEES WERE REMOVING THE CHEEK PLATE FROM THE JAW OF THE CRUSHER WHEN THE LIFTING EYE BROKE. THE STEEL PLATE FELL ON THE JAW AND THEN ONTO THE RIGHT FOOT OF THE EE. PLATE FELL 5 TO 12 INCHES.
