They were adding a pipe to the sand slurry line and the flanges weren't lining up so EE pushed it.

Employees were lifting a pipe using a loader and chain. An employee was adjusting the chain and positioned his body under the loader bucket. The loader operator lowered the bucket to provide slack in the chain pinching the ground employee between the loader bucket and pipe, breaking three ribs.
