WHILE OPERATING LOADER. GLANCED OVER HIS LEFT SHOULDER TO MAKE SURE HE WAS CLEAR. THIS STRAINED HIS NECK TO THE POINT HE COULD NOT STRAIGHTEN WITHOUT PAIN. WAS SEEN BY Occ Medical Services. X-RAY NEGATIVE DX: CERVICAL STRAIN AND ACUTE MUSCLE SPASM. WAS STARTED ON PHYSICAL THERAPY & PLACED ON LIGHT DUTY.
While carrying 5 lb rubber down stairs on tower #2, he missed the bottom step and fell to the ground, striking his right shoulder.

CLEANING OIL, MUD IN CHUTE IN TOWER #1. A CHUNK OF MUD FROM BUILD-UP IN SCREEN FELL ONTO BACK OF NECK. GOING FOR PHYSICAL THERAPY 2-3 WEEKS. DIAGNOSIS: NECK STRAIN AND CONTUSION.
TAKING DOWN CONVEYOR SYSTEM. SLIPPED ON ICE COVERED GROUND, FELL, STRIKING RIGHT ELBOW ON GROUND. AS OF 2/9/04 - NEEDS TO START PHYSICAL THERAPY SESSONS 3 TIMES A WEEK.

EE STATES THAT WHILE WORKING ON TRANSFORMER, BLOCKING SLIPPED AND HE ATTEMPTED TO CATCH IT BEFORE IT FELL. HE DID NOT THINK ANYTHING OF IT AS HE HAD NO PAIN OR EVIDENCE OF INJURY. ON 2/20/03 HE HAD AN ANNUAL PHYSICAL, AN INGUINAL HERNIA WAS DISCOVERED. SAW HIS OWN DR. ON 3/3 AND ON 3/7, REPORTED INJURY ON 2/19 TO HIS SUPERVISOR. SURGERY ON 3/26/03.

HE WAS MIXING POLYMER IN THE FEEDAIR TANK. THE FILL HOSE CAME UNDONE AND HE REACHED TO GET HOSE. HIS FEET SLIPPED UNDERNEATH HIM, DECK WAS WET MIXED WITH POLYMER. HE FELL SIDEWAYS ONTO WATER V ALVING SYSTEM.
EMPLOYEE HAD JUSTN DUMPED MATERIAL. HE PULLED FORWARD WITH THE TRUCK BED STILL RAISED. THE BED GOT CAUGHT ON LOADOUT CONVEYOR BELT. THIS CAUSED THE CAB TO BE PULLED UP OFF GROUND AND JERK BACK DOWN.
