WHILE OPERATING LOADER. GLANCED OVER HIS LEFT SHOULDER TO MAKE SURE HE WAS CLEAR. THIS STRAINED HIS NECK TO THE POINT HE COULD NOT STRAIGHTEN WITHOUT PAIN. WAS SEEN BY Occ Medical Services. X-RAY NEGATIVE DX: CERVICAL STRAIN AND ACUTE MUSCLE SPASM. WAS STARTED ON PHYSICAL THERAPY & PLACED ON LIGHT DUTY.

ON 2/20/03 HE HAD AN ANNUAL PHYSICAL, AN INGUINAL HERNIA WAS DISCOVERED. SAW HIS OWN DR. ON 3/3 AND ON 3/7, REPORTED INJURY ON 2/19 TO HIS SUPERVISOR. SURGERY ON 3/26/03.
