EE was removing a metal panel from the side of a machine for maintenance. EE had EE's hand on the bottom of the panel when EE removed the top bolt. The panel slipped, pinching EE's fingers between the panel and the table.

When re-positioning the stone of the machine he accidentally stepped on the foot pedal causing his thumb to be pinched between the machine and the stone. His left thumb was lacerated at the tip.

Employee's hand slipped while adjusting drill bit
Employee climbed onto the work table to move a stone. While pulling on the stone, a piece broke off causing employee to lose their balance and tumble backwards off the table striking their head.
Employee's duties include sawing, sorting, and packaging stone. According to her report, she lifted a stone from the conveyor and while walking carrying the stone she heard a "pop" in her knee. Immediate medical attention was refused and employee remained at work for remainder of her shift, but she visited the emergency room for treatment the evening of the reported injury.

EE LATER ALLEGES HE WAS EXPERIECING HEADACHES AN D MISSED SOME TIME AND WENT TO DR. EE CONTINUED TO BE SPIRATIC WITH ATTENDANCE. DR NOTIES INDICATED MENTAL ISSUE NEW A PRIOR TO WORKING WITH US.

THE EE WAS SPLITTING STONE WITH A HAMMER. WHILE WING THE HAMMER THE HEAD OF THE HAMMER CAME OFF AND PULLED HIS SHOULDER OUT AND THE HAMMER HEAD HIT HIM IN THE LEG.
THE EE WAS MOVING A PIECE OF STONE FROM UNDER THE GUILLOTINE AT THE END OF THE BELT ANOTHER PIECE OF STONE CAME OFF THE GQUILLTINE BELT PINCHING HIS LEFT PINKY FINGER BETWEEN THE TWO PIECES OF STONE.
EMPLOYEE WAS SPLITTING STONE AND PUTTING THE SPLIT STONE INTO THE DUMPSTER. HE CARRIED A PIECE OF STONE OVER TO PIT IN THE DUMPSTER AND CAUGHT HIS FINGERS BETWEEN THE STONE AND THE DUMPSTER.
WHILE STACKING WALLSTONE AND LOCKING THE STONE BETWEEN (2) PIECES OF STONE WHEN HE GOT HIS FINGER STUCK BETWEEN THE STONE.
EMPLOYEE PICKED UP A PIECE OF STONE TO SET IN THE DUMPSTER AND PINCHED HIS FINGER BETWEEN THE STONE AND DUMPSTER.
EMPLOYEE ALLEGES WHEN LIFTING WALL STONE, HE HURT HIS LOWER BACK HAD TO GET HELP TO LIFT STONE. EMPLOYEE STATES HE DOESN'T THINK IT WAS THAT STONE THAT CAUSED INJURY TO HIS BACK.

THIS INJURY OCCURRED ON 12-5-98 AND AT THE TIME WE DID NOT THINK IT WAS A REPORTABLE INJURY BECAUSE THERE WAS NO LOST TIME AND WE DID NOT THINK THERE WOULD BE ANY FURTHER DR VISITS. HOWEVER, THE EMPLOYEE'S HIP HAS BEEN BOTHERIN GHER AND HAS REQUIRED MORE FOOLOW UP THAN ANYONE ORIGINALLY THOUGHT. EMPLOYEE WAS SORTING STONE FOR STONE SAMPLES, HER FOOT GOT CAUGHT BETWEEN TWO STONES.
EE WAS DRILLING LARGE STONES INTO SLABS FOR THE GUILLOTINE. HE WAS SPLITTING A PIECE OF STONE THAT HE HAD JUST DRILLED AND THE PIECE OF STONE FELL AND SLID DOWN HIS LEG ONTO HIS FOOT.
EMPLOYEE WAS TAILING THE GUILLOTIRE TAKING THE CUT STORES TO TRIM THEM. HE REACHED IN WITH HIS LEFT HAND TO PULL OUT A CUT PIECE OF STONE WHILE ANOTHER STONE WAS BEING CUT, IT SHOT OUT OF THE BUILLOTINE THE SAME TIME HE REACHED IN.
EE STATES THE OTHER OPERATOR ON THE GUILLOTINE WHLE EE WAS TAILING, CUT A STONE AS IT WAS CUTTING A PIECE OF STONE CAME SHOOTING ACROSS THE BELT AT THAT POINT EE WAS PICKING UP A PIECE OF STO NE TO PUT INTO THE DUMPSTER JUST AS HE DID THE SHOOTING PIECE OF STONE CAUGHT HIS FINGER BETWEEN THE 2 STONES
EE WAS PICKING UP STONE, SWINGING BODY TO THE RIGHT WHILE THROWING THE STONE INTO THE DUMPSTER. EE ALLEGED HE HURT HIMSELF WHILE THROWING THE STONE.
EE WAS USING A CROWBAR TO PRY APART A STONE HE HAD SPLIT. A SMALL CHUNK OF STONE FELL AND STRUCK HIS FOOT ABOVE THE STEEL TOE.

EE WAS LIFTING STONE FROM THE GROUND AND PLACINGIT ON A PALLET AND AS SHE WAS BENDING OVER SHEF FELT PAIN IN HER LOWER BACK. EE WORKED MARCH 23,24, 25 A HALF DAY ON 3/26/98 BEFORE SHE REPORTED HER INJURY WHICH SHE SAYS OCCURED ON 3/23/98
EE ALLEGES LOWER BACK PAIN ON 3-18 BUT HE NEVER REPORTED ANY INJURY. HE LEFT WORK AT THE END OF THE DAY ON 3-18 AND REPORTED NO INJURY. HE HAS NOT REPORTED TO WORK, NO HAS HE CALLED IN TO SAY HE WOULD BWE OUT SINCE 3-18; ON 3-24 THE WHITEHAL HEALTH CENTER PHONED STATING EE WAS TGHERE ALLEGING LOWER BAKC PAIN FROM WORKING ON 3-18.
THERE WAS NO ACTUAL ACCIDENT. EE WAS SPLITTING STONE BY HAND USING A 616. AND OR 16 LB. HAMMER HIS FIRST DAY OF WORK HAS 3-9-98 AND AT 12:30 PM THE NEXT COMPLAINED HIS RIGHT WRIST HURT.
