Changing a screen. Prying old screen up and bar slipped and handle struck just above right eye. 1" gash required 3 stitches to close.
Rosk wedged in bottom of hopper door vibrating Grizzly Feeder. One employee was hitting the rock with sledge hammer and a piece of rock broke off and hit another employee in the eye standing adjacent on catwalk causing damage to right eye. Had PPE off to wipe eyes.
Stepping up ladder into Wabco haul truck BD13, when foot slipped on rung. Right knee struck step and ground. Employee had a pre-existing condition with this knee.
CHANGING CAGE ON WHEEL CRUSHER. WHEN TO PULL LAST BOLT FROM HUB ON OLD WHEEL AND DID NOT HAVE CRANE ATTACHED TO TOP OF WHEEL TO IMMOBILIZE IT. WHEEL TURNED WHEN RELEASED AND PINCHED EE'S LOWE R LT. HAND BETWEEN WHEEL-CAGE AND HUB AND IMPACTSOCKET. CRUSHED LITTLE FINGER AND LOWER HAND.
HELPING MOVE CRUSHER WHEEL INTO POSITION, WHEN TWISTED RIGHT KNEE. INJURY BECAME INFLAMED A FEW DAYS LATER.
