Was working on plant. came down steps stepped on ground into low area where rain washed out small area at foot of steps. Turned ankle.

Was welding steel plate for plant, helmet did not fall over fade before arc flashed, burned eyes.

Was cleaning loader for service. Returned to garage to turn off burner on machine to cool off water. Was cleaning concrete with wand when hose stuck on door frame pulling wand into left foot. Water was still hot inflicting burn to left, outside, foot.

EE WAS HOLDING A SQUARE ON REBAR STRUCTURE TO BE WELDED. WITNESS STRUCK AN ARC WHEN THE MIG WIRE DEFLECTED OFF STRUCTURE INTO THIGH. HOT WIRE CAUSED BURN. EE APPLIED FIRST AID & RETURNED TO WO RK. SEVERAL DAYS LATER, THIGH BECAME RED & IRRITATED, SOUGHT MEDICAL ATTENTION. DR. PRESCRIBED ANTIBIOTICS.

TRACKHOE HAD BLOWN AN ORING WHICH RESULTED IN THE NEED FOR ADDITIONAL HYD OIL. EE WAS REMOVING FILLER CAP ON TOP OF MACHINE WHEN HE TURNED AROUND TO DESCEND, LOST HIS BALANCE, MISSED THE HAND HOLD & LANDED ON CRAWLER TRACK BELOW. APPROX 3 FT DROP TO TRACK FROM ABOVE.
