Employee was walking down catwalk and knee popped out of place. This has happened to ee twice in last 6 months in non-workplace setting

Driver impaired and turned vehicle on side when hitting berm.

Employee was in loader taking directions from injured. The loader was lifted up in front, and then dropped. He got out and found injured on the ground.

Laying on back on floor installing a blowbar on a crusher. The blowbar was sitting in a chain hold. The chain slipped and the blowbar fell on his chest and he was unable to remove it. He called to another employee who used a floor jack to lift it. He was taken immediately to the hospital by private car. He had an x-ray and CAT scan of his stomach.

The worker was climbing down off the plant structure and stepped on a rock that was covered by snow when he stepped to the ground.

No snow but mud was slippery. Damage to vehicle not available at this time. Driver was ambulatory but was taken to hospital emergency room.

Blowing concrete off of radiator - fan picked up debris. Blew back while stepping away - got behind safety glasses from underside by cheek bone area.

Employee was jacking up a tire and the jack broke and a piece of the jack hit him in the left hand.

WAS FEELING FAINT SO I STOPPED AT THE BOTTOM OF THE HILL AND SET MY PARKING BRAKE. I LOOKED UP AND EVERYTHING (COLOR WISE) WENT REAL DIM THE TURNED BLACK. NEXT THING I KNW COWORKER IS ASKING M E IF I WAS OK. I WAS IN AND OUT UNTIL I WAS SITTING IN THE CHAIR IN THE OFFICE.

AIR LINE BLEW OFF & HIT EE IN EYE DOWN AT CHEMICAL TRAILER

TRYING TO LOOSEN BOLT WHEN IT HAVE THE FORCE THREW EE OVER THE HAND RAIL & HE FELL 14 FT
