Employee placed his left hand on the outside of the canopy of the scoop that he was operating. The scoop went into a mud hole and slid into a rut smashing his left hand between rib and scoop canopy. After surgery, Dr. said that he would lose 1/2 of his left ring finger and tip of his middle finger.

Foreman as walking the intake airway when a fall was dicovered @ 8:55 am. Location was break 15 #7 heading spad # 324. Fall was 65' long x 18 1/2' wide x 4' thick. Area was dangered off and cribs set at all entrance points. 5' torque tension bolts were installed previously in this area. Falling rock pulled some of the bolts from their anchorage point.

EE was hanging miner cable. When the miner cable started to be pulled, the hook on a rope hanger caught the web of his hand between his thumb and his index finger against the cable causing a cut/laceration on his right hand. Required 6 stitches.

Individual was installing roof bolts when he was struck on the left wrist by a piece of drill steel. He suffered a nondisplaced hairline fracture to his left wrist. He did not cease work to his left wrist. He did not cease work and was treated later and placed in a wrist splint.

The employee was unhooking a piece of pipe connected to the bucket of an excavator when the operator engaged the emergency stop lever so he could move the bucket. The operator didn't realize his leg was depressing the bucket joystick. The bucket dropped onto the employee's left hand, pinching his fingers. Surgery was performed & ee is off work.

Employee states that he was tramming up an incline with a beam on the forks of a forklift when the forklift stopped running causing brakes, steering & hydraulics to fail.
