Yellowbush Mine Coal

Employee was told to pull a loose rib that had timbers set in front of it. When employee pulled the loose rib, the material dislodged a timber striking employee in the head.

Fall was found on 2 south mains at 5 and 6 break #2 entry 1 break outby spad #421. Fall was 5' to 6' thick x 60' long x 14' wide. Roof support was 6' torque tension bolts.

Fall was found at 12 break on 2 south mains #8 entry, 60' inby spad 495. Fall was 55' long x 7 to 8' thick, and 18.5 ' wide. Roof support was 5' torque tension bolts.

Roof fall was found on 2 south mains 12 break #6 entry 60' inby spad #485. Fall was 8' thick x 19' wide x 60' long. Roof support was 5' torque tension bolts.

Fall was found on 1st left off of south mains, inby spad #954. Fall was 18' long x 18' wide x 4' to 5' thick. Roof support was 6' torque tension bolts.

Employee placed his left hand on the outside of the canopy of the scoop that he was operating. The scoop went into a mud hole and slid into a rut smashing his left hand between rib and scoop canopy. After surgery, Dr. said that he would lose 1/2 of his left ring finger and tip of his middle finger.

Fall was found off of 1 west mains old track entry 5 break. Fall was 5' thick x 20' long x 20' wide. roof control was 5' torque tension bolts.

Fall was found on 1st left off of 1 west mains old track entry. Fall was 75' long x 9' thick x 20' wide. 14 break track entry. roof support was 5' torque tension bolts.

Fall was found on 28 break, 1 west mains, 6 entry and 6 right crosscut. Roof support was 6' torque tension bolts.

Fall was found on 1 West mains 4 left panel #2 entry 9 and 10 break. Roof control was 6' torque tension bolts.

Employee was trying to remove a shear jack pin. Employee was holding a bar when he was struck with a hammer in the right forearm, fracturing a bone in his arm.

Fall was found 2nd left off 1st left panel off south mains 2-room. The fall was 70'long x 19' wide x 5' to 6' high. Roof support was 6' torque tension bolts.

Fall was found 23 break, 1 west mains 7 right crosscut. Fall was 15' long x 5' to 5 1/2' thick x 18 1/2' wide. Roof support was 5' torque tension bolts.

Fall found 1st left panel, south mains #1 rooms spad # 231. Fall was 70' long x 19' wide x 6' high.

Fall was found at 45 break on 1st left off of south mains, 2 breaks inby spad # 1662. Fall was 30' long x 19' wide x 5' to 6' thick. Roof support was 6' torque tension bolts.

Installing torque arms on miner felt pain in stomach. Employee filled out accident form but refused medical attention, and continued work until 11/10/11. Employee had surgery on 1/11/11.

4 left panel off of 1 west mains, #9 entry, 7 break. Dimensions were 35' long x 6' to 7' thick x 18.5' wide. Roof control was 6' torque tension bolts.

Fall was found 1st left off of 1 west mains break 28, track entry spad # 568. Dimensions were 12' wide x 25' long x 6' thick. Roof support was 5' torque tension bolts.

Fall was found on 4 left panel off of 1 west mains, between 4 break and 6 break #9 entry. Spad #1521 in #8 entry across from fall. Dimensions were 60' long x 7' to 8' thick x 19' wide. Roof support was 6' torque tension bolts.

Fall was found at 1st left off 1st left off 2 south mains 61/2 break to 10 break #2 entry near spad 1202. Fall was 160' long x 19' wide x 8' thick. Roof support was 6' torque tension bolts.

Fall was found at 1st left off 2 south mains at 1 crosscut inby spad 1058 to 1055 no.2 intersection. Roof support was 6' torque tension bolts.Fall was 100' long x 20' wide.

Roof fall was found off of 1 west mains 2 left panel #1 entry. Roof fall was approx. 55' long x 19' wide x 7' thick.

Fall was found off of #2 south mains inby spad # 1024. Fall was approx. 25' wide x 20' long x 6' to 7' high.

Roof fall was found on 1 west mains 20 break, #6 entry in the intake. spad # 355. Fall was apppox. 25' long x 19' wide x 5' thick. Roof control was 5' torque tension bolts.

Fall was found at 1 west mains intake #8 entry.Fall was 40' L x 19' W x 5' thick.8 left demensions were 25' L X 19'W X 5' Thick.

Fall was located on 1 west mains 9 break #9 entry. Fall was 20'W x 20'L x 5' Thick.

Fall was found at 1 west mains #3 entry intersection. Fall was approximatly 30'L x 19.5' W x 6' H.

Roof fall was found on 1st left off 1 west sub mains, 18break-20break #4 entry, spad # 469. Fall was 110' long 19' wide 7' thick.

Roof fall occurred on 2 south mains 2nd left break 27 in the belt entry. Fall was 20' x 20' x 8' thick. Roof control was 6' torque tension bolts.

Fall was found at 1 west mains, 25 break, spad # 382. Fall was 80' long x 19' wide x 7' high. roof support was 5' and 6' torque tension bolts.

Fall was found on 2 south mains #8 entry inby spad # .777 from 35 break to 36 break. Roof control was 6' torque tension bolts. Fall was 120' long x 20' wide x 8' high.

Fall was found on 2nd left off 2 south mains at 24break. Roof fall was 6' High x 40' Long x 18'Wide.

Fall was found on 1 west mains #9 entry break 32 1/2 to 34 break. Roof Fall was 7' Thick x 18' Wide x 90' long.

Fall was found at 1 west mains, 12 break between #4 and #5 entry, 50' inby spad #301. Fall was 15'W x 20'L x 5'H

Operator had left hand on canopy jack while lowering canopy. His left pinky got smashed between cylinders.

Roof fall was found at break 38 on west mains 3 breaks inby spad #1240, between #6 and #8 entry. Fall was 17.5 W x 8' H x 70' L .

Fall was found at break #5 on 2nd left panel from spad 811 to 361. Fall was 55' long, 18' wide, and 5' thick.

Fall was found in intake on 2 south mains, inby spad # 758 # 7 entry break 32. Fall was 60' long 20' wide approx. 10' high.

Fall was located on 1B belt at break # 14 outby spad # 322. Fall was 25' long 19' to 25' wide 5' high.

Roof fall was found at 1 west mains 2nd left panel belt entry, break 2, 1st crosscut outby spad 354 at end of belt channel.

Fall was found 1 west mains break 40 4 to 6 entry. Fall was 80' long x 18' wide x 7' thick.

Roof fall was found at 9:10pm in the return on 1 left panel in 20 south mains at break # 41. Fall started in the intersection of #7 entry and went approximately 150' thru the intersection in # 9 entry from spad 2245 to spad 1166. This area was mined around the first of June. Area was dangered off.

Fall was located in # 2entry on South Mains in the return, inby break 14 to 15 1/2. Fall was 120'long x 18' wide x 5' thick. Roof support was 5' torque tension bolts. MSHA inspectors investigated the fall.

EE was checking a pump at 1 west mains when he discovered a fall at 37 brk entry 7,8,9 in the xcuts. Roof fall is inby spad # 1271, # 9 entry. Roof fall is approx. 100' long X 19' wide X 8' to 9' thick. Fall has been dangered off.

Foreman as walking the intake airway when a fall was dicovered @ 8:55 am. Location was break 15 #7 heading spad # 324. Fall was 65' long x 18 1/2' wide x 4' thick. Area was dangered off and cribs set at all entrance points. 5' torque tension bolts were installed previously in this area. Falling rock pulled some of the bolts from their anchorage point.

Roof fall was at #3 x-cut on 2 left west mains. Fall was 18'w x18'L x 8' to 9' thick. Two cribs were set at all approaches to fall. Area had been bolted with 5' torque tension bolts. and spotted with 8' bolts. Fall was in neutral 1st x-cut outby spad # 339. Area was last mined October of 2009.

EE was hanging miner cable. When the miner cable started to be pulled, the hook on a rope hanger caught the web of his hand between his thumb and his index finger against the cable causing a cut/laceration on his right hand. Required 6 stitches.

Roof fall was at x-cut 11 in 1 West Mains in No. 4 entry. Fall was 20'W x 20'L x 5' Thick. Area was bolted with 5' torque tension bolts with A3 resin. Area was mined in 09/09.

Roof fall was at x-cut 30 in 1 west mains between no. 6 and No. 8 entries. Fall was 20'W x 100'L x 5' Thick. Set posts at all approaches to fall. Area was bolted with 5" torque tension bolts with A3 resin. Area was mined in 10/09.

Roof fall at No. 4 entry at spad 257 on 1 south mains. Fall was 18'L x 20'W x 2'5' thick, did not clean up fall-area dangered off. Fall was in a non mining area and no airway blockage. Area was bolted with 5' torque tension bolts.

Roof fall at No 2 entry between 20 & 21 x-cut on 1st left panel off of 1 west mains. Fall was 60' L x 20' W x 5' thick. Did not clean up. Cribbed & dangered off. Fall was outby section. No airway blockage. Area was bolted with 5' torque tension bolts.

Individual was installing roof bolts when he was struck on the left wrist by a piece of drill steel. He suffered a nondisplaced hairline fracture to his left wrist. He did not cease work to his left wrist. He did not cease work and was treated later and placed in a wrist splint.

Roof fall at No 5 entry between 15 & 16 x-cut on 2 south mains. Fall was 20' wide X 50' long X 5'-9' thick. Cleaned up fall and rebolted area. 10 day reporting window extended due to mine holiday and vacation.

Roof fall at No. 11 & 12 x-cut, No. 1 entry of 1st left panel off of 1 west mains. fall was 20'W x 70'L x 3'-6' Thick. Cribbed and dangered off area, will not be cleaned up. Area bolted with 5' torque tension 3/4" roof bolts. Immediate roof was shale with laminated sandstone above it. Fall inby loading point.

Roof fall at No. 11 x-cut, No. 9 entry of 1 west mains. Fall was 20' W x 35' L x 3' - 5' thick. Cribbed and dangered off. Will not clean up. Area bolted with 5' torque tension 3/4" roof bolts. Immediate roof was shale with laminated sandstone above it.

EE was hanging a water line when he slipped and struck his head on the rib. EE suffered a laceration to the right side of his head, which required 4 sutures.

EE states he was in a squatting position grinding metal, when he felt back pain and hip pain when standing. EE sought medical treatment on 7/2/09 and was taken off work by his treating physician.

The employee was unhooking a piece of pipe connected to the bucket of an excavator when the operator engaged the emergency stop lever so he could move the bucket. The operator didn't realize his leg was depressing the bucket joystick. The bucket dropped onto the employee's left hand, pinching his fingers. Surgery was performed & ee is off work.

Employee states that he was tramming up an incline with a beam on the forks of a forklift when the forklift stopped running causing brakes, steering & hydraulics to fail. The forklift rolled backwards, caught traction with tires & proceeded to roll over on it's top. Employee suffered head contusion.

Employee states he felt low back pain while he was unloading a truck bed.

EE was assisting in the installation of a jib on a 90ton crane, while standing on a ladder, the jib fell off the crane and onto the ladder, knocking the ladder over and the employee fell to the ground, suffering a contusion to his lower back and tailbone.

Employee states he was getting steel beams ready for crane lift and felt low back pain/left leg pain when he bent over and lifted end of beam. He delayed seeking medical attention to see if pain would ease alone. Condition did not improve and treatment was sought 10/24/08.
