Struck against a moving object

Company haul truck driver was found crossways on the haul road with truck facing a retention berm. EMS was notified and driver was air lifted to Wichita Falls Tx for treatment and care. American Gypsum was notified and they notified MSHA. Unknown cause of accident. It was reported by EMS personnel that employee had stated they had blacked out.

Struck against a moving object

On August 21, 2018 the equipment operator in the quarry dept serviced and inspected the company truck #380 and the employee stated the truck was on good working order. Afterwards the equipment operator was driving the company vehicle to the pit area to deliver diesel. Operator went off the berm into pit. EE hit head on the frt window causing a 3-4"" laceration on forehead.

Removing hydraulic lines and brackets from parking brake. The hose and bracket fell downward pinning employee's arm against the steering cylinder causing employee to strain shoulder

5/12 EE HELPED SHOVEL MAT'L DUE TO CRUSHER CONVEYOR JAM. FROM 8AM TO 11:20AM & FROM 4:30PM TO 5:20PM 5/13 EE RAN WATER WAGON FOR APPROX 2 HRS. STATED THAT HE FELT IT JOLT HIM. 5/14 EE AWOKE WITH TINGLING/NUMBNESS IN HIS ARM & HAND. DIAGNOSED AS DEGENERATIVE BULGING DISCS AGGRAVATED BY SHOVELING & OPERATION WATER WAGON

Fall from machine

ON THE MORNING OF 4-4-01 EE REPORTED THAT HE SLIPPED AND FELL WHILE CLIMBING ONTO A CAT 777B HAUL TRUCK. HE REPORTED THAT HE HIT HIS CHEST ON THE STEPS, SLIGHTLY KNOCKING THE WIND OUT OF HIM, AND ALSO JARRED HIS BACK WHEN HE HIT THE GROUND.
