Removing a conveyor belt punch from a drill and it slipped and cut two fingers on the left hand. Stitches were required to close cuts on thumb and middle finger.

Was looking at rollers and stepped into an offset hole made by the loader tire.
HANDS WERE WET AND LOST GRIP WHILE ATTEMPTING TO GET IN LOADER.
PULLED ON FLUME WHILE FEEDING HOPPER.
POURING AN 80LB BAG OF SAND INTO ANOTHER CONTAINER, THE BAG SLIPPED AND HE ATTEMPTED TO GRAB IT AND HURT HIS WRIST.
SWUNG BAG AROUND AND BACK POPPED.
