Using an angle grinder to prep the scalper lip for welding. During the process of grinding, the employee's safety glasses slid down EE's nose which resulted in debris going in EE's left eye.

Operator performing a screen change, when the head of the bolt spun resulting in laceration to left index finger. Required 7 stitches. Full Duty Release.
