Using an angle grinder to prep the scalper lip for welding. During the process of grinding, the employee's safety glasses slid down EE's nose which resulted in debris going in EE's left eye.

Operator performing a screen change, when the head of the bolt spun resulting in laceration to left index finger. Required 7 stitches. Full Duty Release.

Operator checking on a pump not discharging. Shut power off and removed the fill plug to wash out the mud. Hot slurry blew out and hit wrist, chest, neck and chin. Resulting in 2nd and 1st degree burns. Released to modified duty and given a prescription.

Changing drill bit in drill. Finger got caught in key and chuck. 1/22 limit use of right hand, wearing splint. Fracture to right index finger.

Using hand held grinder, wearing safety glasses. Grinding 1/4"" steel.

Setting down hinged end of conveyor. While hooking up to it, employee walked up the belt end tipped back toward him and pinched his foot.

MAINTENANCE ON CRUSHER. 18"" PIPE WRENCH KICKED BACK WHEN PRESSURE APPLIED. FACIAL CONTUSION/BRUISE.

Flame cutting a metal piece with cutting torch and a piece of slag fell and dropped into lap and burned inner right thigh and penis.
