WORKER WAS TIGHTENING BOLTS ON EQUIPMENT AND ON HIS WAY DOWN THE CATWALK LOST HIS BALANCE AND FELL.
WORKING ON JAW CRUSHER. ATTEMPTING TO LOWER WEDGE TO RESET OPENING. WEDGE BOUND UP. EMPLOYEE HIT IT WITH SLEDGE HAMMER TO MAKE IT DROP SEVERAL TIMES. IN FINAL ATTEMPT INJURY INCURRED. WED GE DROPPED SUDDENLY WHILE WORKER WAS TURNING NUTBY HAND. LITTLE FINGER WAS PINCHED BETWEEN NUT & SURFACE WHERE NUT RESTS.
