Using an angle grinder to prep the scalper lip for welding. During the process of grinding, the employee's safety glasses slid down EE's nose which resulted in debris going in EE's left eye.

EE WAS BRACING SIDE BOARD WITH A 5 LB SLEDGE HAMMER TO STRAIGHTEN METAL RIBBING THAT HELPS HOLD SIDE BOARDS IN PLACE. ABOUT THE 5-6TH SWING HEAD OF HAMMER SNAPPED OFF STRIKING EE ON LEFT SIDE OF NOSE.
