Operator performing a screen change, when the head of the bolt spun resulting in laceration to left index finger. Required 7 stitches. Full Duty Release.

Operator checking on a pump not discharging. Shut power off and removed the fill plug to wash out the mud. Hot slurry blew out and hit wrist, chest, neck and chin. Resulting in 2nd and 1st degree burns. Released to modified duty and given a prescription.
