Employee twisted back when stepping down out of the bulk truck. Later Shoveling dirt when putting off the shoe, aggravated it even more.

Spontaneous combustion

Employee was stepping down off machine and hand slipped off the handrail, heel caught on the track and EE fell backwards and hit back on the opposing track.

Prying with pry bar, bar slipped and employee fell.

At end of shift, in the process of beginning to fuel equipment, employee slipped and fell approximately 10 feet to the ground.
