Employees were replacing protective wing shroud on the left side of the bucket on the 6020 excavator. While attempting to put the shroud in place, the shroud dropped unexpectedly pinching EE's fingers between the shroud and edge of the bucket. The shroud weighing approximately 40 pounds fell 10 inches before striking the employee's left 3rd finger causing fracture and laceration.

Had left hand resting on the steel side of the rock picking table when an approximately 200lb rock fell down from the feeder striking employee's hand. No initial treatment needed. Later, numbness developed and recommended surgery. Did not have knowledge of this until 7/8/25.

Employee was dismounting a 777 haul truck on mounted ladder located on the non-cab side. EE took a step onto the bumper ladder and when EE did, the steps swung, causing EE to throw off EE's balance. EE's right hand slipped off the ladder, spraining EE's middle finger.
Employee was placing machinery teeth on a pallet to be moved with the skid loader. A tooth fell off and landed on the ground and employee decided to pick up the 100+ pound tooth and place it back on the pallet while the skid loader was still in motion. Employee felt a pop in lower back and had instant pain

The employee was working in the Logan breaker picking breaker rock. While unwedging a large rock that became wedged on the picking table between the divider and the side of the table, the employee put left hand on the side of the table to brace self near the feed chute and a rock came in and hit the tip of the ring finger
Employee was climbing down the ladder of a CAT 777B Haul Truck at end of shift. EE missed placing EE's right foot onto the last step and EE's right foot slid to the ground, causing left shoulder to hold EE's body weight. EE's right hand slid off ladder because EE was in the process of taking the step and was removing EE's right hand to slide it down the rail.
Removing a guard on the push arm of a 375A bulldozer to make a minor repair using a wrench. Wrench slipped and struck employee on right shin causing a laceration.
Employee was dismounting the D10 dozer at the end of EE shift. While utilizing steps and per Employee "3 points of contact", EE slipped off step located on blade arms, landing on EE's Left Side.
Employee was carrying a case of water down to cleaning plant and EE slipped on fresh snow/ice landing directly on back, knocking the wind out of EE. Employee continued to finish out the shift picking rock. On 1/31 EE left work early to seek treatment because still in pain from contusion.

Standard Threshold Shift. Hearing loss.
Employee was in the process of placing the protective cap on an empty acetylene cylinder. When EE went to tighten the cap, it became stuck and employee tried to jar it loose. While in the process, the cylinder started to tip over and pinched finger between the cylinder and storage cap. Caused laceration to left middle finger requiring 2 stitches.

Broken side cutter was removed from demag bucket with welding truck crane. Hook-on cable got welded in point sheave. Welding partner lowered winch causing slack on the reel. Employee yanked on broken piece which came down on his finger because of slack.

Employee arrived at the mine to find his co-worker in sudden cardiac arrest. Employee attempted resuscitation efforts unsuccessfully. Employee suffered a mental health emergency from this incident.
Employee suffered a sudden cardiac arrest secondary to an acute myocardial infarction while seated in the office trailer at this mine. Employee was a security supervisor at this mine. Employee had a past medical history of cardiac illness.

Employee has discovered apparent bite marks, both red and bumpy in color, similar to that of a mosquito bite. Employee has found these apparent bites on both forearms and on the #4 finger on the left hand.
Employee contacted our emergency on duty manager and presented with speech problems. Employee asked for help to be dispatched. Employee was working second shift security.
Said he was lifting a rock and felt pop in shoulder. Reported to foreman but said he needed no treatment. *On 2/6/13 he reported to foreman he needed to seek treatment for shoulder.*

Moving T-4 drill and flipped drill on it's side.
Employee injured himself on 7/13/07 but did not seek treatment. He went to the doctor a few weeks later and has been off work since 09/06/07.
