EE WAS DOING GENERAL CLEANUP WORK WHEN HE FELT PAIN IN LOWER BAKC AND DOWN HIS RIGHT LEG. EE HAD AN HERNIATED DISK.

EMPLOYEE WAS CUTTING A METAL PLATE OUT OF A FEEDER BOX WITH A CUTTING TORCH. HE WAS RESTING HIS BODY ON THE PLATE. WHEN THE PLATE FELL, THEREFORE THE HOT PLATE STRUCK HIM ON THE SHOULDER, LE FT KNEE, AND LOWER RIGHT LEG. THE EMPLOYEE WAS NOT FOLLOWING NORMAL PROCEDURES REQUIRED FOR THE PROJECT.

EMPLOYEE ATTEMPTED TO START THE PLANT WATER PUMP WHEN HE RECEIVED AN ELECTRICAL SHOCK THROUGH THE MAGNETIC STARTER. HOSPITALIZED ONE NIGHT FOR OBSERVATION. EMPLOYEE WAS TRAINED AND ALL PROTECT IVE DEVICES WERE ON EQUIPMENT. FAULTY STARTER DUE TO LIGHTNING DAMAGE.
