Contractor finished drilling hole, started to lower mast, pinched little finger between the mast & window frame of operator's cab. Resulted in the amputation of his little finger.

Rock truck had lost brakes. went down slope where top soil was being applied. Ran over 2 top soil piles and bed then rolled over.
