Employee and another man were carrying piece of stone tread. He was walking backwards, tripped over another piece of stone, fell & twisted right knee.

Employee requested relief from operating loader. He came up to office where he felt dizzy, nauseous, & head ached. He vomited, passed out and fell onto floor. Ambulance transported him to hospital.

Employee was helping load stone treads onto a customers truck. While carrying the piece to truck, he felt his right knee pop.

Injured EE and another EE were pulling stone treads for an order. They removed 1 piece from pallet and leaned it against pallet in order to pull next piece. The leaning tread fell on top of injured EE's foot.

Employee was pulling ""stone treads"" for cutting orders. He lifted a heavy tread & pulled a stomach muscle.

Employee was loading baskets with building stone products when he strained the muscles in his right wrist & forearm.

EE was rolling 2' boulder of end of the ""forics"" of the forklift, lost his grip and boulder smashed his right hand. Received 8 stitches.

Employee chipping off edge of large concrete block with a sledge hammer when a piece of steel or concrete went under safety glasses & hit his eye.

EE was lighting a burn pile (broken wood pallets) and got to close to the frame. He burned his right arm & right side of face.

EMPLOYEE WAS PULLING OUT PIECES OF BLUESTONE TREAD FOR AN ORDER WHEN HIS THUMB BECAME STUCK BETWEEN TWO PIECES.

EMPLOYEE WAS LOADING STONE ONTO A CUSTOMER TRUCK WHEN HE SMASHED HIS FINGER BETWEEN BED OF TRUCK & THE ROCK.

PLACING STONE TREADS IN STOCK PILE WHEN HIS FINGER BECAME CAUGHT BETWEEN TWO.

EMPLOYEE WAS HAND UNLOADING BLUESTONE WHEN ONE PIECE FELL OUT OF PALLET AND ONTO HIS FOOT.

EMPLOYEE WAS HAND PICKING STONE FROM A STOCKPILEWHEN ONE FELL ONTO HIS HAND.

WHILE EMPLOYEE WAS TRIMMING TREES, HE FELL OUT ONTO GROUND APPROX. 15'.

WHILE DRIVING FORKLIFT, HIT BUMP, BOUNCED AND HIT HEAD.

MOVING PIECE OF STONE, PULLED MUSCLE IN SHOULDER.

WHILE TORCHING BLUESTONE WITH PROTECTIVE EYEWEAR IN PLACE, HOT STONE CHIP FLEW INTO EYE.

EE WAS FLIPPING A TREAD OF STONE WHEN IT CAME DOWN ON HIS FINGERS.

EE WAS LIFTING A TREAD OF STONE WHEN IT STARTED TO DROP. HE TRIED TO CATCH THE PIECE OF STONE & PULLED HIS ARM/ELBOW IN THE PROCESS.

EE WAS LIFTING STONE TREADS ALL DAY & BACK MUSCLES TIGHTENED AT NIGHT.

FLIPPING TREADS OF STONE OVER AND CAUGHT FINGER UNDERNEATH.

EMPLOYEE OPENED A BOTTLE OF STONE SEALER WHICH SPLASHED UP AND OUT OF CONTAINER AND INTO HIS EYE.

EMPLOYEE WAS TOSSING STONE PRODUCT INTO BUCKET OF FRONT END LOADER WHEN HE SLIPPED , WRENCHINF HIS SHOULDER AND BACK.
