Employee requested relief from operating loader. He came up to office where he felt dizzy, nauseous, & head ached. He vomited, passed out and fell onto floor. Ambulance transported him to hospital.
Injured EE and another EE were pulling stone treads for an order. They removed 1 piece from pallet and leaned it against pallet in order to pull next piece. The leaning tread fell on top of injured EE's foot.
EMPLOYEE WAS TOSSING STONE PRODUCT INTO BUCKET OF FRONT END LOADER WHEN HE SLIPPED , WRENCHINF HIS SHOULDER AND BACK.
