Individual was installing a roof bolt when EE was setting the head of the bolt into the chuck and hit the rotation. EE's glove unknowingly got caught between the bolt head and the chuck resulting in stitches.

A fire occurred at the shop/office building during a non-work shift when no employees were present at the mine. It is unknown what caused the fire, however the building and its contents were a complete loss. The fire chief on site believed that the fire started on the west side of the building.

Mine foreman was helping the roof bolters in cross cut #3-4. EE was bending 6' bolts and helping the roof bolters. A piece of scale rock fell out of the roof between the rib and the bolts knocking hard hat off. A 2nd piece of scale rock fell out striking ee on top of the head, cutting head. This resulted in stitches.

The area was supported with 6 foot, fully grouted resin rods. The area was dangered off. The fall did not affect the ventilation at this mine.

The area was supported with 42 inch fully grouted resin rods. This area was dangered off. The fall did not affect the ventilation at this mine.

Area was bolted with 36" fully grouted resin bolts according to the approved roof control plan. Area was timbered and dangered off. Did not affect any equipment or ventilation.

An unplanned roof fall occurred in the 8 Left Rooms area, 1 crosscut inby Station number 4642. This was discovered during a weekly exam of this area. The fall was approximately 6 feet high, 19 feet wide, and 70 feet long. Normal ventilation was not disturbed. The affected area was timbered and dangered off.

WHILE EE WAS DRILLING A HOLE TO INSTALL A ROOF BOLT THE WRENCH SLIPPED OFF THE DRILL STEEL. WHEN THE STEEL CAME OFF IT STRUCK THE EE ON THE FOREHEAD RESULTING IN A SMALL CUT REQUIRING 4 STITCH ES BUT DID NOT CAUSE ANY LOST TIME TO HIS NORMALDUTIES.
