EMPLOYEE WAS OPERATING A 482 DIESEL SCOOP OUT-BY ON THE INTAKE TRAVELWAY AND FELT A PULL IN THEIR BACK.

Contributing factor mine is old 1997 and had 36" bolts.

Employee was in an underground shop and hit self on the head. EE was alone.

Individual was installing a roof bolt when EE was setting the head of the bolt into the chuck and hit the rotation. EE's glove unknowingly got caught between the bolt head and the chuck resulting in stitches.

A fire occurred at the shop/office building during a non-work shift when no employees were present at the mine. It is unknown what caused the fire, however the building and its contents were a complete loss. The fire chief on site believed that the fire started on the west side of the building.

Changing a grease cartridge in a grease gun. Pulled back on the handle and felt something pull in neck.

Mine foreman was helping the roof bolters in cross cut #3-4. EE was bending 6' bolts and helping the roof bolters. A piece of scale rock fell out of the roof between the rib and the bolts knocking hard hat off. A 2nd piece of scale rock fell out striking ee on top of the head, cutting head. This resulted in stitches.

Dry conditions at the face. Employee was building a return wall. Block fell off of top row and hit finger resting on top of another block.

EE was grinding with a dewalt 4 1/2" grinder. The grinder kicked striking ee on left hand resulting in 5 stitches.

Low coal seam 38" and area is dry. Lifting a part for the miner employee felt a strain in employee's left shoulder.

We have a 38" coal seam. Employee was crawling on the mine floor with knee pads.

Employee was crawling on the bottom turned head and caught left ear on the end of a roof mat.

Employee was pulling a Johnson buggy into the shop and the tires were muddy. One of the tires picked up the rope that was hooked to the garage door rolling it around the tire. Door was pulled down onto employee's back.

On 4/21/2015 two employee's were cleaning up rock with a shovel the one employee was hit by the shovel in the right pointer finger. Resulting in a scratch. On 4/27 2015 employee was admitted to the hospital for an infected finger.

Hit head on ATRS required 12 stitches

Muddy and wet just out-by the face

While putting in low belt structure he twisted and felt something pull in his lower back and hips.

Employee was going thru a mandoor. He caught corner edge of door with his right eyebrow. Causing laceration.

He was installing a 6' roof bolt. He was trying to bend it in the hole. The bolt spun and he drove his shoulder into the bolter.

Employee was working with a 1/2 drive ratchet and a socket.The socket slipped off of the bolt and he drove his hand into the bottom breaking a small bone in his hand.

He was changing a tire on a scoop with chains. Reached around the back side of the tire and cut his finger on a sharp burr.

Draw rock fell on top of the miner. Employee was getting it off with a rock bar and twisted his back.

Roof bolting @ the face, bottom was muddy and wet. Leg from knee down to foot were sucked into the mud. He twisted his upper body and his knee popped.

The fall was in the old works not affecting travel or ventilation. Return Rooms of 2 south mains #1 and #2 room at spad #3422 outby MP-6 80' wide x 60' long x 6' high.

While he was helping to install a belt drive he was climbing on the drive and twisted his knee.

Crew was belting up they were pulling the belt tight 1/2" chain broke and tail kicked and hit employee in the leg.

While making a splice on a belt, the belt clamp came off and hit him in the teeth.

The fall is in old works - Not affecting travel or ventilation. Location - 3 N butt, left side room #14 from the face back 270'. 6' high.

Bad roof area, 3 North Butt, #27 Room - left side. Affecting travel but not airflow.

Employee was pulling some draw rock off of the miner when another piece of rock slid against his right hand cutting and smashing his fingers.

While bolting EE turned around to move back to his controls and twisted his knee.

EE was cutting a piece of conveyor belt with a razor knife when the knife slipped cutting his hand.

EE crawled on top of a rock, injuring his knee.

Employee was tightening bolts on the Joy miner and felt something pop in his arm, shoulder & elbow.

EE was crawling through a muddy area and twisted his knee.

EE was crawling around scoop in charging station and rubbed against corrugated tin, cutting his arm. Received 5 stitches in arm at Indiana Hospital ER.

While crawling past his scoop, EE's arm rubbed against a steel wicker sticking out of frame of scoop, cutting his arm. Laceration required 5 stitches.

Employee got his finger caught between 2 low belt rails while belting up.

While pushing a 42" roof bolt into the roof with the head of the bolter, the bolt slid off to the side, pinching the EE's hand.

Roof fall in return room off 2 south mains from #2 Entry through Intersection to #1 Entry. Roof fall was around 70' in length. This fall was in the old works and there was no damage to any equipment. The area was cribbed and dangered off.

While pulling the MBC ahead during a move, a rock came out of the rib of a high spot striking employee on the foot.

Employee was roof bolting when a piece of draw rock fell striking his elbow.

EE was pulling in his work boots when injury occured.

Roof Fall in first left in #4 entry around 6600' from loading section of 3 south mains, Roof Fall was around 50' in length, 6 feet in height. EQ involved and one piece of high voltage was replaced and rebolted after cleanup.

While pulling the low belt tight with a scoop, the chain broke, letting the tail fly back and strike EE in the hand.

EE was building cribs. Another EE was handing him crib blocks and his pinky finger was smashed between two blocks.

Employee was lifting rocks out of his MBC comveyor chain and one slid against his hand cutting and fracturing his finger.

While adding drill steels, employee wanted to swing bolter head to clear away loose rock and coal to gain more ground clearnace. Instead of hitting swing lever, he accidently hit up lever.

Employee was roof bolting when a piece of rock fell and struck him in the arm.

Employee was getting ready to bolt a cut when he experienced excessive knee pain.

Employee was installing short rails on low tail, when the tail was released, EE's finger was caught between the short rails and the low belt tail.

While loading bags of cement into a hopper of a cement mixer, the cement blew back up into his eye.

EE was in the process of drilling a roof bolt hole. When he was adding the pusher or driver steel, his thumb got caught in the steels.

Employee was installing a 4' resin roof bolt into the hole, when a rock fell from roof striking and crushing his thumb.

An unplanned roof fall was discovered in the return air course in a room panel off 10 Right Main.This fall is located at spads #5241 and 5242. It is approximately 10 feet high and 60 feet long. The area was supported with 6 foot, fully grouted resin rods. The area was dangered off. The fall did not affect the ventilation at this mine.

An unplanned roof fall was discovered in the return air course in a room panel off 11 Right Main. This fall is located 60 feet inby spad #5323. It is approximately 10 feet high and 200 feet long. The area was supported with 42 inch fully grouted resin rods. This area was dangered off. The fall did not affect the ventilation at this mine.

Employee was roof bolting when a drill steel fell out of roof striking employee on left hand.

An unplanned roof fall was found at approximately 10:30A.M. in the #2 entry of 1st Left, 1 x-cut outby survey station #81. Fall was approximately 4' high, 19' wide and 10'-12' long. Area was bolted with 36" fully grouted resin bolts according to the approved roof control plan. Area was timbered and dangered off. Did not affect any equipment or ventilation.

Employee was operating a continuous miner when he was struck by coal from the pressure of an uncharted drill hole.

Employee was drilling roof to install roof bolt when a piece of rock dislodged from roof striking employee on left side of head causing laceration of left ear and contusion of left leg. Employee did not miss any work however he did receive sutures making this a reportable.

An unplanned roof fall occurred in the 8 Left Rooms area, 1 crosscut inby Station number 4642. This was discovered during a weekly exam of this area. The fall was approximately 6 feet high, 19 feet wide, and 70 feet long. Normal ventilation was not disturbed. The affected area was timbered and dangered off.

Employee was bending a Roof Bolt in low coal so as to install in the roof when he experienced pain in middle of back along the right side. Employee was placed on restricted (modified) duty effective 11/07/2006.

While performing his normal duties as a scoop operator employee experienced pain in right knee. Employee came back to work on his next scheduled shift and continued working until October 10, 2005 when he started missing work due to this injury.

EE WAS IN CLOSE PROXIMITY OF A COWORKER WHO WAS HAMMERING ON A PIN WHEN A METAL SLIVER BROKE OFF, FLYING INTO EE'S LEFT EYE.

While performing his normal duties as a supervisor in low coal; employee experienced swelling in the left knee area. Employee continued working until 6-8-05 at which time he started missing work.

WHILE EE WAS OPERATING MBC THE CABLE GOT STUCK ON RUBBER MAT. WHILE TRYING TO FREE CABLE UP EE TWISTED LEFT KNEE. EE CONTINUED TO WORK UNTIL 03/30/05 AT WHICH TIME EE STARTED MISSING WORK.

After positioning roof bolter to drill along. the rib the ee inadvertently set stab jack down of right foot.

WHILE PERFORMING HIS NORMAL DUTIES AS A ROOF BOLTER / MBC OPERATOR, EMPLOYEE EXPERIENCED PAIN WHILE CRAWLING IN LOW COAL. EMPLOYEE RETURNED TO WORK ON HIS NEXT SCHEDULED SHIFT HOWEVER HE STARTED MISSING WORK ON 10/6/04 MAKING THIS REPORTABLE.

EE, WHILE SPREADING HIGH CALCIUM LIME, APPARENTLY GOT SOME ON THE KNEE AREA OF CLOTHING NOT REALIZING THAT. LATER ON DURING THE SHIFT HE GOT HIS KNEES WET WHICH CAUSED A CHEMICAL REACTION.

While performing his normal duties as a mechanic in low coal, ee experienced pain in left knee. EE continues to work at this time but was placed on restricted duty starting 7-29-04, now making it reportable.

WHILE EMPLOYEE WAS DIGGING ROCK IN COAL CHAIN, ROCK SMASHED HAND LACERATING FINGERS. EMPLOYEE RECEIVED 3 SUTURES AND PUT ON RESTRICTED DUTY, MAKING THIS A MEDICAL.

When ee went to get off personnel carrier he experienced pain in left knee. EE continued working until July 20, 04 at which time he started missing work, now making this a lost time accident, scheduled for surgery.

Ee had just swung the drill head on the fletcher RRII roof bolter to drill hole for sister hook when his SCSR hit the swing lever thus pinching his knee to the coal rib from the drill head of the bolter.

EMPLOYEE WAS USING AN AIR IMPACT WRENCH TO TAKE BELT STRUCTURE APART WHEN WRENCH APPARENTLY HAD THE NUT OFF PERMITTING SOCKET END TO STRIKE EMPLOYEE IN RIGHT EYE.

While checking roof employees hammer slipped striking EE in the mouth chipping his tooth. He went to the dentist on 03/17/04 to get work done making this report a medical.

While performing his normal duties as a scoop operator, EE experienced pain in rt.knee. EE has not missed any work however was placed on restricted duty starting 03/16/04. Therefore making this a medical report at this time.

After crawling 7,800 feet out of mine due to fan outage, EE had swelling in knee.

WHILE PERFORMING NORMAL DUTIES AS A MINE EXAMINER (THROWING ROCK DUST ETC.) EE AGGRAVATED RIGHT ELBOW. EE CONTINUED WORKING UNTIL 2/18/04 AT WHICH TIME HE STARTED MISSING WORK.

CHANGING BITS ON MINER - KNOCKING BROKEN BIT OUT OF INSERT WHEN HE STRUCK HIS LEFT HAND WITH HAMMER PUNCTURING HAND. RECEIVED SUTURES THUS MAKING THIS REPORT A MEDICAL.

AN UNPLANNED ROOF FALL OCCURRED IN THE #2 ENTRY OF 6 RIGHT SECTION AT SURVEY STATION #3816. FALL WAS APPROXIMATELY 5' HIGH, 25' WIDE, 30' LONG. AREA WAS TIMBERED AND DANGERED OFF.

AN UNPLANNED ROOF FALL OCCURRED IN THE #1 ENTRY OF EAST MAINS AT SURVEY STATION #1251. FALL WAS APPROXIMATELY 6' HIGH, 30' LONG, 20' WIDE. FALL WAS TIMBERED TO GAIN ACCESS TO THE AREA.

EE WAS BENDING A BOLT PRIOR TO INSTALLATION WHEN HE FELT A PAIN IN HIS BACK.

EMPLOYEE WAS CHANGING BATTERIES ON THE SCOOP IN THE CHARGING STATION WHEN THE PLUGS APPARENTLY STUCK. WHILE TUGGING ON PLUGS THE ONE PLUG SUDDENLY DISLODGED THUS STRIKING HIM ON UPPER LIP.

WHILE EMPLOYEE WAS LOOKING FOR AN OIL LEAK ON A ROOF BOLTER HE EXPOSED HIS LEFT HAND IN A PINCH POINT THEN OPERATED FUNCTIONS PINCHING LEFT MIDDLE FINGER.

EE WAS KNOCKING OUT OLD CONCRETE WALL WHEN BLOCK DISLODGED FROM TOP ROW STRIKING EE ON 3RD FINGER OF LEFT HAND. INJURY WAS CRUSHING TYPE ALONG WITH A LACERATION REQUIRING STITCHES, THEREFORE MAKING THIS REPORTABLE.

WHILE EMPLOYEE WAS CRAWLING IN LOW COAL HE EXPERIENCED PAIN IN RIGHT KNEE. EMPLOYEE CONTINUED WORKING UNTIL 8/14/03 AT WHICH TIME HE WAS PLACED ON RESTRICTED DUTY.

WHILE PERFORMING HIS NORMAL DUTIES AS A MINE EXAMINER IN LOW COAL - EE EXPERIENCED PAIN IN RIGHT KNEE. EE CONTINUED WORKING UNTIL 7/7/03 AT WHICH TIME HE SAW A PHYSICIAN AND WAS PLACED ON REST RICTED DUTY.

EE EXPERIENCED PAIN IN HIS RIGHT KNEE WHILE CRAWLING. HE CONTINUED WORKING UNTIL SEPT. 29, 2003 AT WHICH TIME HE STARTED MISSING WORK IN ORDER TO HAVE SURGERY DONE.

EE WAS SCOOPING THE BOTTOM OF THE #3 ENTRY IN 5 RIGHT ROOMS WHEN HE STRUCK HIS HEAD AGAINST THE ROOF.

WHILE EE WAS ROOF BOLTING HE EXPERIENCED PAIN INKNEE. EE CONT'D WORKING UNTIL 4/29/03 AT WHICH TIME HE STARTED MISSING WORK. NOT MAKING THIS REPORT A LOST TIME ACCIDENT

A ROOF BOLT WAS HANGING OUT OF ROOF, WHAT WE TERM A "CHANDELIER" WHEN THE EE STRUCK HIS RIGHT EAR ON THE BEARING PLATE THAT WAS ON THAT ROOF BOLT CAUSING LACERATION. EE RECEIVED SUTURES, THERE FORE MAKING THIS REPORT A MEDICAL.

EE WAS HELPING CO-WORKERS SET "I" BEAMS AND CRIBS WHEN HE EXPERIENCED PAIN IN LEFT KNEE AREA. EE CONTINUED WORKING UNTIL MAY 27, 2003 AT WHICH TIME EE STARTED MISSING WORK TO HAVE SURGERY PERF ORMED ON THAT SAME KNEE.

WHILE EE WAS ROOF BOLTING A HYDRAULIC HOSE FOR THE ROTATION BLEW SPRAYING HYDRAULIC OIL ONTO THE FACE AROUND GLASSES AND INTO BOTH EYES. EE WAS TAKEN TO LOCAL HOSPITAL WHERE MEDICINES WERE USE D IN THE EYES MAKING THIS REPORT A MEDICAL.

WHILE CARRYING A BAG OF ROCK DUST, EE FELL, INJURING RIGHT KNEE. EE CONTINUED WORKING UNTIL 1-14-03 AT WHICH TIME HE STARTED MISSING WORK TO HAVE SURGERY DONE TO REPAIR THE TEAR.

AN UNPLANNED ROOF FALL OCCURRED IN THE #3 ENTRY (BELTLINE) OF 3 LEFT. FALL STARTED AT SURVEY STATION #2751 AND TRAVELED INBY 2 X-CUTS TO JUST INBY SURVEY STATION #2753. FALL WAS APPROXIMATELY 6'-8'HIGH, 19'WIDE, 100'LONG. AREA BOLTED WITH 4' FULLY GROUTED RESIN BOLTS ACCORDING TO THE APPROVED ROOF CONTROL PLAN.

EMPLOYEE WAS REMOVING A PIECE OF ROCK FROM THE BELT WHEN HE SUSTAINED A LACERATED FINGER THAT REQUIRED SUTURES.

EE WAS BENDING OVER TO PICK UP A ROOF BOLT WHEN HE EXPERIENCED PAIN IN LOWER BACK.

WHILE EE WAS INSTALLING COAL CHAIN ON MINER HE EXPERIENCED PAIN IN LEFT SHOULDER. EE DID NOT MISS ANY WORK UNTIL DECEMBER 3, 2002 AT WHICH TIME HE HAD SURGERY PERFORMED.

EMPLOYEE WAS LOADING DRILL HEAD FROM A ROOF BOLTER ONTO A PERSONNEL CARRIER WHEN THE DRILL HEAD SLIPPED PERMITTING THE SPROCKET TO CATCH ON HIS GLOVE PULLING ON HIS RIGHT SHOULDER. EMPLOYEE ST ARTED THERAPY ON 6/26/02 NOW MAKING THIS REPORT A MEDICAL.

EE MOVED TO PUT BOLT IN AND WHEN DOING SO EXPERIENCED PAIN IN LOWER BACK. EE STARTED MISSING WORK ON 6-25-02. MINES IDLE FROM 6-29-02 UNTIL 7-7-02.

EE WAS CLEANING ROCK OFF THE TOP OF MINER WHEN A PIECE OF ROCK FELL OFF THE CONTINUOUS MINER STRIKING THE EE'S LEFT KNEE. EE CONTINUED WORKING UNTIL 7/9/02 AT WHICH TIME HE STARTED MISSING WOR K.

EE WAS ROOF BOLTING - WHILE DOING SO HE SCRAPED HIS BACK ACROSS A BEARING PLATE CAUSING A LACERATION THAT REQUIRED SUTURES.

EE WAS PULLING CHARGER PLUG & CABLE OVER TO PERSONNEL CARRIER TO PUT ON CHARGE WHEN HE EXPERIENCED PAIN IN LOWER BACK.

WHILE EE WAS ROOF BOLTING AT THE FACE AREA, HE STRUCK HIMSELF ON THE LEFT KNEE WITH A DRILL STEEL. FOUR DAYS LATER, THE KNEE WAS STILL SORE AND HE DECIDED TO SEEK MEDICAL ATTENTION. AT WHICH T IME THE DR. INSTRUCTED HIM TO STAY OFF HIS KNEE FOR 2 DAYS AND IF IT FELT BETTER, HE COULD RETURN TO WORK, WHICH HE DID. OFF WORK-1-17-02 & 1-18-02.

WHILE CRAWLING EMPLOYEE STRUCK LEFT KNEE ON A PIECE OF ROCK. EMPLOYEE DID NOT MISS ANY WORK BUT WAS PLACED ON RESTRICTED DUTY, EFFECTIVE 11-8-01.

EMPLOYEE WAS RE-BUILDING A BLOCK WALL WHEN ONE OF THE BLOCKS DISLODGED PREMATURELY AND LANDED ON RIGHT HAND.

AN UNPLANNED ROOF FALL WAS FOUND BY THE SUPERVISOR WHILE MAKING HIS AIR COURSE RUN AT THE DARMAC #2 MINE. THE FALL OCCURRED IN THE #3 ROOM OF THE LAST ROOM SET-UP TO THE LEFT OFF THE MAIN ENTR IES BEFORE DARMAC #2 CEASED PRODUCTION. FALL WAS LOCATED AT SURVEY STATION #2608 INTERSECTION. FALL WAS APPROX. 4 1/2' HIGH 20' X 20'. AREA WAS BOLTED WITH 48" RESIN BOLTS. AREA WAS DANGERED

AN UNPLANNED POWER OUTAGE OCCURRED AT OUR DARMAC#2 OPERATION WHICH TOOK OUR FAN OUT. POWER OUTAGE WAS CAUSED DUE TO A BLOWN TRANSFORMER. MEN WERE WITHDRAWN FROM THE MINE PER THE APPROVED FAN S TOPPAGE PLAN. POWER WAS RESTORED AT APPROX 1:30 PM IN THE SAME DAY SPECIAL EXAM WAS CONDUCTED AFTER NORMAL WATER GAUGE

EE WAS STANDING APPROX. 20' FROM A QUICK DUSTER WHEN THEMECHANIC WHO WAS WORKING ON THE DUSTER STARTED IT UP TO TEST RUN WHEN APPAREHTLY A PARTICLE OF DUST/DIRT FLEW OUT AND WENT INTO EE'S LEF T EYE.

WHILE EE WAS SHOVELING LOOSE COAL, EE EXPERIENCED PAIN IN LOWER BACK. EE CONTINUED WORKING UNTIL 1/21/97 AT WHICH TIME THIS BECAME A LOST TIME ACCIDENT.

EE WAS OPERTING BATTERY POWERED SCOOP TRACTOR-EESTRUCK HIS HEAD ON A BELT HANGER WHICH RESUTLTEDIN A PULED MUSCLE IN HIS NECK.

THE EMPLOYEE WAS ROLLING UP A 110 VOLT EXTENSION CORD AT THE SURFACE AREA FOR THE DARMAC #2 DEEP MINE. WHILE HE WAS ROLLING UP THE EXTENSION CORD HE TRIPPED AND FELL STRIKING HIS RIGHT LEG AND KNEE.

WHILE EMPLOYEE WAS REMOVING AND LOADING BELT STRUCTURE ALONG BELT LINE HE EXPEERIENCE D PAIN IN HIS LOWER BACK. HE COPLETED HIS SHFIT BUT BY HIS ENXT SCHEDULED WORK DAY HE REQUESTED TO SEEK M EDICAL TREATMENT WHICH RESULTED IN LOSS OF WORK DAYS TO GIVE THE LOWER BACK MUSCLES TIME TO HEAL.

WHILE EE WAS DRILLING A HOLE TO INSTALL A ROOF BOLT THE WRENCH SLIPPED OFF THE DRILL STEEL. WHEN THE STEEL CAME OFF IT STRUCK THE EE ON THE FOREHEAD RESULTING IN A SMALL CUT REQUIRING 4 STITCH ES BUT DID NOT CAUSE ANY LOST TIME TO HIS NORMALDUTIES.

WHILE EE WAS CRAWLING AROUND THE SECTION & GETTING IN & OUT OF HIS BATTEY POWERED SCOOP TRACTOR HE EXPERIENCED PAIN IN HIS RIGHT KNEE.

WHILE EMPLOYEE WAS INSTALLING ROOFBOLTS WIHT A ROOFBOLTING MACHINE, THE WRENCH SLIPPED OFF THE HEAD OF THE ROOFBOLTS WHILE HE WAS TIGHTENING THE BOLT RESULTING IN THE EMPLOYEE SPRAINING HIS WR IST.

EMPLOYEE WAS DRIVING OUT THE HINGE PINS ON A BELT DRIVE WHICH WAS OUTSIDE THE MINE BUT WAS BEING TORN APART IN ORDER TO TAKE INTO THE MINE. WHILE EMPLOYEE WAS SWINGING A SLEDGE HAMMER, THE HEA D OF THE HAMMER CAME OFF CAUSING THE EMPLOYEE TO PULL A MUSCLE IN HIS SHOULDER.

EE WAS INSTALLING ROOF BOLTER WITH A LONG AIR BOX ROOF BOLTING MACHINE, AFTER INSTALLING A BOLT HE FELT PAIN IN HIS RIGHT KNEE. THE EE REPORTED IT THAT SHIFT TO HIS FOREMAN BUT DID NOT SEEK ME DICAL TREATMENT TILL 8-2-95. AT WHICH TIME HE WAS INSTRUCTED TO STAY OFF OF IT TILL FURTHER MEDICAL OPTIONS COULD BE DECIDED.

WHILE EMPLOYEE WAS BREAKING BANDS ON A BUNDLE OFROOFBOLTS THE BAND BROKE AND THE EMPLOYEE SLIPPED AND FELL ON A BUNDLE OF ROOF BOLT PLATES STRIKING HIS LOWER BACK CAUSING A SPRAIN IN HIS BACK.

EE WAS OPERATING A SINGLE BOOM ROOF BOLTING MACHINE IN THE 3 ENTRY OF THE WORKING SECTION AFTER DRILLING THE HOLE HE TWISTED AROUND TO GET A ROOF BOLT FROM THE ROOF BOLTER HELPER AND FELT A PA IN IN HIS LOWER BACK RESULTING FROM A PULLED MUSCLES.

EMPLOYEE WAS OPERATING A BATTERY POWERED SCOOP TRACTOR IN THE EAST MAIN SECTION OF THE DARMAC #2 MINE. WHILE TRAMMING FROM THE #3 ENTRY TO THE X-CUT 3-2 , HIS HEAD STRUCK A SMALL OFFSET IN THE MINE ROOF CAUSING HIS HEAD TO BE JAMMED DOWNWARD & RESULTING IN A SPRAIN TO THE NECK AND BACK. EMPLOYEE DID NOT REPORT ACCIDENT TIL 1-4-94 AT WHICH TIME HE WENT HOME EARLY AND SAW DOCTOR.

EE WAS FABRICATING STEEL. HE TOSSED A SMALL PIECE OF STEEL ONTO THE WORKOUCH AND MINUTE PIECE OF STEEL BOUNCED UP FROM THE BENCH. IT LANDED INTHE EE S LEFT EYE.

MINE HAS BEEN IDLE SINCE 2-01-93. CAVE IN X-CUT 3-2, AND #2 ROOM, 1 SO. ROOMS. CAVE APPROX. 18' LONG 15' WIDE, 4-1/2' TO 5' THICK. AREA HAD 30", 36" & 48" RESIN BOLTS INSTALLED. THERE WAS A CL AY VEIN IN THIS AREA. CAVE HAPPENED SOMETIME BETWEEN 10-14-93 & 10-20-93.

EMPLOYEE WAS INSTALLING ROOF BOLT, PUTING PRESSURE ON BOLT FOR RESIN TO SET UP, WHEN THE MOUNT FOR THE STAB JACK BROKE AND CANOPY CAUGHT EMPLOYEES LEFT HAND SMASHING 2 & 3RD FINGERS ON LEFT HA ND.

EMPLOYEE WAS HELPING MECHANIC WELD ON MINER PARTS WHEN HE RECEIVED A WELDING FLASH.

EMPLOYEE HAD JUST INSTALLED BOLTS, WHEN THE HELPER DROPPED THE CANOPY TO MOVE BOLTER, A PIECE OF ROCK SLID OFF THE ATRS AND STRUCK EMPLOYEE ON BACK.

EMPLOYEE CRAWLING IN LOWCOAL IRRITATED LT KNEE AND THIS BECAME INFECTED.

WHILE BOLTING XCUT 2-1 A PIECE OF DRAW SLATE 4FTX3FTX6IN FELL & STRUCK EMPLOYEE ON LEFT FOOT

EMPLOYEE WAS DRILLING HOLE IN ROOF. 1 PIECE OF ROCK DISLODGED AND CAUGHT MIDDLE FINGER LEFT HAND BETWEEN THE ROCK AND HEAD OF ROOF BOLTER.

EMPLOYEE BENDING ROOF BOLTS FOR INSTALLATION & IN DOING THIS HE FELT PAIN IN BACK & LEGS

EMPLOYEE WAS SHOVELING A COAL SPILL ON TO THE BELT AND FELT A SHARP PAIN IN HIS LOWER BACK.

TRAMMING BRIDGE FROM FARE OF 2 ENTRY GOT SQUEEZED BETWEEN BRIDGE AND RIB OF 3.

EMPLOYEE TWISTED HIS LEFT KNEE WHILE PUTTING A XOB OF RESIN ON TOP OF ROOF BOLTER.

EMPL MOVING ROOF BOLTER FROM AREA OUTBY FACE AND PIECE OF ROCK FELL ALONG RIB AND STRUCK HIM ON LEFT SHOULDER AND LEFT FOOT.
