3 employees working on elev. platform to remove pitman from Crusher B. Crane owned & operated by contractor had lifted pitman & it was hanging stationary. The men headed for the stairs to descend. Pitman suddenly swung and hit EE, fatally injuring him (cause of death crushing blunt force trauma). 2nd EE was grazed by pitman, but was not injured. 3rd EE uninjured.
EE was working off a ladder adding steel on the hopper and the high reach jerked causing EE to fall off ladder onto arms.
Conditions at the shot were good. Employee climbed over rock pile and toppled over on the opposite side of the pile.
Employee was climbing down off of conveyor belt (2-3' off the ground). His foot slipped off side, twisting and causing a fracture to left foot when his foot hit the ground.
DRILLING HOLES FOR BLASTING; DRILL STEEL WAS STUCK. STRIKER BAR BROKE; A PIECE OF STEEL HIT EMPLOYEE IN THE FACE.
WORKING ON CHUTE UNDER CRUSHER. AREA UNDER CRUSHER WAS CAGED IN. EE OPENED CAGE DOOR AND WALKED IN STOOD UP, WALKED INTO BAR.
