While swinging a sledgehammer at a wrench, the wrench slipped off the nut, causing the handle to come back and strike the left side of the EE's face.

EE coughed a felt a pop in EE's right groin.

EE slipped while climbing into their truck and slammed their left foot into the ground.

EE had a steel plate slip out of EE's grip and strike EE's right leg.

EE grabbed a spinning bolt with bare hands and cut EE's left middle finger.

Individual fell from a platform. Suffered a broken lower leg.

EE went out to the quarry to lay out a shot. He slipped on rocks & fell, hurting left knee. Then, while walking into the office, EE walked into a well pipe that was sticking out of the ground & hurt left knee even more.

CLIMBING LADDER TO CRUSHER, SLIPPED, FELL, PUT HAND OUT TO BRACE FALL.

HELPING TO CHANGE BACKHOE BUCKET PUTTING SPACER WASHERS IN WHERE PIN GOES, HE TOLD THE OPERATOR TO HIT THE PIN IN FURTHER. THE PIN WENT IN EASY AND PINCHED HIS FINGER.

CLIMBING UP INTO CAB SLIPPED ON STEP ICEY.

HAUL TRUCK WAS LOADED ON TOP BENCH, COMING DOWN HAUL ROAD MADE TURN ON THE RAMP SLID ON MUD AND ROLLED OVER.

RUNNING TO SHUT OFF FEEDER BECAUSE BELT HAD SHUTOFF SLIPPED IN MUD.

DIRECTING LOADER INTO POSITION. STEPPED BACK INTO MUD AND SAND UP OVER BOTH KNEES. WHILE TRYING TO GET OUT PULLED BACK OUT.
