Employee on simplicity screen cat-walk cleaning with a digging bar wearing gloves. Employee hit a finger w/digging bar onto a angle iron on cat walk. Yard Manager called MSHA per report. Found paper report not submitted at time of filing 2nd Qtr. 7000-2 form.

Skid Steer operator, while backing up with a functional alarm sounding, struck the plant operator, resulting in leg and ankle/foot injuries to the plant operator.
