EMPLOYEE TOOK THE AIR TANK OUT OF HIS TRUCK DUE TO FROZEN WATER. WHILE PLACING THE TANK BACK INTO THE TRUCK, EE SLIPPED ON THE LADDER CAUSING HIS LEFT SHOULDER TO HIT THE TRUCK CAUSING THE INJ URY. THE TANK WEIGHED APP 25 LBS, AND THE HEIGHTTO CLIMB ON THE LADDER WAS APP 6 FT.

INJURED IS NOT SURE EXACTLY WHEN IT HAPPENED BUT HE HAD BEEN GRINDING ROGUGH EDGES OFF OF PAVING FORMS,HE HAD HIS SAFETY GLASSES ON.THOUGHT SOME DUST HAD BLOWN INTO HIS EYE.IT DIDN'T BOTHER HI M UNTIL THE FOLLOWING DAY,HE WENT TO PHYSICAN AND HAD IT CHECKED.
I NAME WAS WASHING OUT WITH A WASH HOSE IN THE FEEDER TRAP AREA HE ACCIDENTLY DROPPED THE WASH HOSE & IT WAS FLIPPING AROUND ON THE FLOOR WHILE TRYING TO RETRIEVE THE HOSE FROM UNDER THE FEED ER PLATE HE PLACED HIS LEFT HAND ON A PIECE OFANGLE & THE FEEDER PLATE CAME BACK & PINCHED HIS THUMB

A PIECE OF STEEL WENT AROUND HIS GOGGLES AND GOT IN HIS EYE WHILE HE WAS GRINDING A PIECE OF METAL HE DID NOT GO TO THE HOSPTIAL TO HAVE IT REMOVED RIGHT AWAY AS IN DID NOT BOTHER HIM.
