Struck by flying object

EE was breaking jasper brick grinding media with a sledge hammer in order to obtain smaller pieces. EE busted a piece that flew up and cut middle finger on left hand. EE supervisor controlled the bleeding and took ee to the hospital where EE received two stitches.

Over-exertion (Not Elsewhere Classified)

The operator was performing the task of placing a tarp over a shaker screen. EE was standing on a pedestal to access the screen. EE slipped while attempting to step down from the pedestal to the catwalk and hyperextended left knee. The operator went to the doctor at the end of the shift. Nothing was broken but EE was given a knee immobilizer and released to return to work.

Re-bricking mill and while placing bricks inside mill, he pinned right pinky finger between bricks and mill door

Employee attempting to bore a hole with a cordless drill when the drill bit broke causing employee to lurch forward at which point he cut the thumb of his free hand with the portion of drill bit still remaining in the drill.

Fall to the walkway or working surface

On third level of plant, ran into a low clearance area, fell back and hit tail bone on catwalk.

Cleaning up the discharge blower chute on the Mill. EE attempted to empty the discharge flap, failed to notice a bunge cord that was securing the flap closed and pushed the flap open which caused the bunge cord to break and slip free from the flap striking EE in the lower nose and upper lip area.

Strained/pulled muscles in lower back while emptying 50 lb bags of product. Went to Custer Regional Medical Emergency Room, Custer, SD.

EE WAS AT THE EXPLOSIVES MAGAZINE. HE WAS TRYING TO OPEN A JAMMED DOOR (THE WOOD ON THE BACK OF THE DOOR EXPANDED DUE TO MOISTURE). HE STRAINED HIS LOWER BACK. HE REPORTED THE INCIDENT BUT DID NOT SEEK TREATMENT. ON 5/22 HE WAS BARRING ORE IN THE CRUSHER PIT AND REINJURED HIS BACK. HE IS NOW BEING TREATED BY A CHIROPRACTOR & MAY BE SCHEDULED FOR FURTHER MEDICAL TREATMENT.

BENT TO SLIDE AND LIFT A BOX OUT OF A VEHICLE AND PULL OR STRAINED BACK MUSCLES. PROGRESSIVELY GOT WORSE BY NEXT DAY. HE MISSED NO TIME OFF WORK. HE WENT TO LAMMERS CHIROPRACTIC CLINIC ON 12/6 /01, 12/7/01 AND 12/10/01.

Struck by flying object

EE WAS WALKING ACROSS PLANT AREA. WIND BLEW A FOREIGN OBJECT INTO HIS RIGHT EYE. IT DID NOT BOTHER HIM UNTIL LATER IN THE EVENING AT WHICH TIME HE WENT TO THE EMERGENCY ROOM AT THE CUSTER HOSP ITAL. HE MISSED NO TIME OFF WORK AND RETURNED TOWORK THE NEXT DAY AT THE BEGINNING OF HIS SHIFT. HE WAS WEARING SAFETY GLASSES AT THE TIME OF THE ACCIDENT.
