Fall to the walkway or working surface

The employee was walking to a piece of equipment in the early morning hours before full sunlight when they stepped on a rock, rolling their ankle which caused them to fall to the ground.

EE was replacing battery while standing on operator platform of equipment when they rotated with battery in hand causing their foot to slip off the platform and going between the track and engine compartment body. EE was initially released to full duty and later placed off work by the medical provider.

EE was changing screens and slipped hitting shin on a piece of metal causing a laceration requiring 12 staples.

EE was tightening a belt on the plant screen and the wrench slipped off the nut and EE slipped, and while trying to catch themselves, hit arm on roller on plant and broke the left arm

Croell Redi-Mix employee, was changing a track and rollers on dozer and when lying underneath trying to reposition roller he twisted his body and felt pain in lower back and tail bone.

Employee was changing screens at Wasta Pit and pulled muscle. Went to doctor and is a possible hernia. Going back to doctor on 08/07/2008 to confirm. Questionable. Off work from date of injury until hernia is confirmed and if surgery is needed.

While running gradation for sand and gravel samples strained her back. This mine was not operational. Only use trailer for sampling purposes. Went to doctor and had a work release until 3-31-05. Was released on 3-31-05 with restrictions of wt lifting equal or less than 25 pounds until recheck on 4-11-05.
