An employee lost their footing while installing an access door on a chute. While attempting to prevent a fall, the employee used their left arm to brace themselves while still holding an impact wrench. The resulting force caused a fracture to the employee's left elbow.

LEFT FRONT TIRE HIT THE REAR TIRE ON DUMP TRUCK, THEN LEFT MIRROR HIT THE BED AND BROKE IT OFF. STEERING DAMAGED.

DR.wants to see patient again. scripts Norco for the pain at night and Etodolac for inflammatory.

He thinks he may have hurt it during March 2009 while washing and cleaning the chutes at the primary. According to the management team, no one was aware of this incident until July 2009.

AFTER HE MOVED THE MACHINE AWAY FROM OIL STORAGE TA NKS, THE HOT OIL CAME OUT THE FILLER TUBE LOCATED BEHIND THE SEAT. THE HOT OIL BURNED THE OPERATOR ON THE BACK AND ARM.
