Employee was reaching for tools next to a pallet of VSI shoes and one of the shoes fell off of the pallet and struck the top Rt hand of the employee.

STATED THAT LOADER HE WAS OPERATING WAS GOING DOWN AN INCLINE AND THE ENGINE QUIT. HE JUMPED FROM THE MACHINE CAUSING THE INJURY.
