Employee got a foreign body in left eye. The foreign body was picked up by fan when hitting bumps in road.

PUTTING A SIGN POST IN THE GROUND, USING A LOADER BUCKET. THE SIGN POST HIT SOLID ROCK, AND THE POST BOWED AND SLIPPED AWAY FROM THE LOADER AND HIT EE'S UPPER RIGHT ARM WITH FORCE.
CLIMBING DOWN FROM EXCAVATOR WALKWAY ONTO A LADDER. LADDER FOOT SANK INTO MUD CAUSING IT TO TIP.EMPLOYEE FELL ABOUT 4 FT TO THE GROUND.
EE HAD JUST FINISHED A SAND SAMPLE ANALYSIS AND NOTICED AN IRRITATION OF HIS RT. EYE. EE TRIED TO FLUSH IT OUT AND COULDN'T. WENT TO DR. & HAD FOREIGN OBJECT REMOVED & USED ANTI-BIOTIC OINTME NT SEVERAL TIMES A DAY TO PREVENT INFECTION.

DRAGGING WASH HOSE TO CONVEYOR #1. AS HOSE UNFOLDED A HOSE CLAMP PUNCTURED LEFT FOREARM.
INSTALLING TAIL SHAFTS ON NORTH LOG WASHERS, WORKING IN VERY AWKWARD POSITIONS, PUSHING AND PULLING HEAVY OBJECTS INTO POSITION AND TIGHTEN-ING BOLTS.
